摘要
目的 探讨临床鉴别诊断胸腹水良恶性分别采取细胞学检查、DNA定量分析、细胞块石蜡切片技术的诊断价值。方法 选择2022年1月-2022年6月我院接诊治疗的胸腹水患者171例作为研究对象,分别对其进行细胞学检查、DNA定量分析、细胞块石蜡切片诊断,将病理诊断结果作为金标准,计算三种诊断技术鉴别诊断胸腹水良恶性的灵敏度、特异度与准确率,比较三种技术联合诊断与单一技术诊断的灵敏度、特异度、准确率统计学差异。结果 病理诊断显示,在171例胸腹水患者中,83例患者为良性胸腹水,占比48.54%,88例患者为恶性胸腹水,占比51.46%。计算结果显示,三项技术联合诊断灵敏度、特异度、准确率均高于单一诊断(P<0.05)。结论 临床鉴别诊断胸腹水疾病可选择细胞学检查、DNA定量分析、细胞块石蜡切片三种技术联合方案,可有效提高准确率,为临床提供数据支持,提倡运用推广。
Objective To explore diagnostic value of cytological examination,DNA quantitative analysis,and cell block paraffin section techniques in differential diagnosis of benign and malignant hydrothorax and ascites in clinical practice.Methods The paper chose 171 patients with hydrothorax and ascites treated in our hospital from January 2022 to June 2022 as research objects.Cytological examination,DNA quantitative analysis,and cell block paraffin section diagnosis was performed respectively.With gold standard of pathological diagnosis results,sensitivity,specificity,and accuracy of three diagnostic techniques was calculated for distinguishing benign and malignant hydrothorax and ascites.Statistical difference of sensitivity,specificity,accuracy of combined diagnosis of three techniques and diagnosis of single technique was compared.Results Pathological diagnosis of 171 patients with hydrothorax and ascites showed that 83 patients had benign hydrothorax and ascites,accounting for 48.54%,and 88 patients had malignant hydrothorax and ascites,accounting for 51.46%.Calculation results showed that sensitivity,specificity,and accuracy of combined diagnosis of three techniques was higher than single diagnosis(P<0.05).Conclusion Combination of cytological examination,DNA quantitative analysis,and cell block paraffin section can be used for clinical differential diagnosis of hydrothorax and ascites diseases,can improve accuracy,provide data support effectively for clinical practice,which is worthy of applicati on and promotion.
