摘要
目的:分析超声联合增强型血流成像技术对胎儿心脏畸形的诊断效能。方法:选取2019年1月—2020年12月郑州市妇幼保健院产科300例接受产前检查的孕妇作为研究对象,均给予二维超声、增强型血流成像技术检查,以随访至分娩结果为参照,分析两种方法单一应用与联合应用的诊断效能差异。结果:300例孕妇随诊证实存在胎儿心脏畸形的有53例,胎儿心脏畸形发生率为17.67%。单一二维超声、单一增强型血流成像技术与两者联合诊断的准确度分别为69.67%、81.00%、89.67%,诊断敏感度分别为41.51%、73.58%、90.57%,诊断特异度分别为75.71%、82.59%、89.47%,漏诊率分别为58.49%、26.42%、9.43%,误诊率分别为24.29%、17.41%、10.53%,阳性预测值分别为26.83%、47.56%、64.86%,阴性预测值分别为85.78%、93.58%、97.79%。单一增强型血流成像技术的诊断效能均优于单一二维超声,两者联合诊断的准确度、敏感度、阳性预测值、漏诊率、误诊率优于两种方法单一诊断,差异有统计学意义(χ^(2)=4.859、6.821、7.949、7.135,P<0.05),联合诊断的阴性预测值、特异度高于单一二维超声诊断,差异有统计学意义(χ^(2)=3.668、4.974,P<0.05),但与单一增强型血流成像技术诊断,差异无统计学意义(P>0.05)。结论:超声联合增强型血流成像技术诊断胎儿心脏畸形可获得较单一方法诊断更高的准确度、敏感度,可有效减少漏诊、误诊率,可为临床进行产前胎儿心脏畸形的诊断提供更多指导。
Objective:To analyze the diagnostic efficacy of ultrasound combined with enhanced flow imaging for fetal cardiac malformations.Methods:A total of 300 pregnant women undergoing antenatal checkups in hospitals from January 2019 to December 2020 were selected for this study,and all were given 2D ultrasound and enhanced blood flow imaging techniques.The difference in diagnostic efficacy between the single application and the combined application of the two methods was analyzed with reference to the results of the follow-up to delivery.Results:The presence of fetal cardiac anomalies was confirmed in 53 of 300 pregnant women at follow-up,with an incidence of fetal cardiac anomalies of 17.67%.The accuracy of single 2D ultrasound,single enhanced flow imaging technique and combined diagnosis of both were 69.67%,81.00,89.67%,diagnostic sensitivity was 41.51%,73.58%,90.57%,diagnostic specificity was 75.71%,82.59%,89.47%,missed diagnosis rate was 58.49%,26.42%,9.43%,misdiagnosis rate was 24.29%,17.41%,10.53%,positive predictive value was 26.83%,47.56%,64.86%,and negative predictive value was 85.78%,93.58%,97.79%,respectively.The diagnostic efficacy of both single enhanced flow imaging techniques was superior to that of single 2D ultrasound,and the accuracy,sensitivity,positive predictive value,missed diagnosis rate,and misdiagnosis rate of the combined diagnosis of both were better than those of the two methods alone,with statistically significant differences(χ^(2)=4.859,6.821,7.949,7.135,P<0.05).The negative predictive value and specificity of the combined diagnosis were higher than those of the single 2D ultrasound diagnosis,and the difference was statistically significant(χ^(2)=3.668,4.974,P<0.05),but the difference was not statistically significant when compared with the single enhanced flow imaging technique diagnosis(P>0.05).Conclusion:Ultrasound combined with enhanced flow imaging technique can achieve higher accuracy and sensitivity in the diagnosis of fetal cardiac malformations than the single application of one
