摘要
目的分析对剖宫产术后子宫瘢痕妊娠孕妇采用超声引导下清宫术治疗的临床疗效。方法选取60例剖宫产术后子宫瘢痕妊娠孕妇,根据单双号法将其随机分为对照组(采用米非司酮治疗)与试验组(采用超声引导下清宫术+米非司酮治疗),每组30例。比较两组孕妇围术期指标、术后2个月并发症发生率、月经恢复正常率、血人绒毛膜促性腺激素(HCG)下降至正常时间及附件包块完全吸收时间。结果试验组术中出血量、住院费用少于对照组,住院时间、手术时间短于对照组(P<0.05)。术后2个月随访中,试验组并发症发生率显著低于对照组(3.33%VS 26.67%),月经恢复正常率高于对照组(96.67%VS 73.33%)(P<0.05)。试验组血HCG下降至正常时间、附件包块完全吸收时间短于对照组(P<0.05)。结论对剖宫产术后子宫瘢痕妊娠孕妇采用超声引导下清宫术治疗的效果显著,值得临床推广。
Objective To analyze the clinical eflFect of ultrasound-guided uterine clearing for pregnant women with uterine scar after cesarean section.Methods 60 pregnant women with uterine scar after cesarean section were selected and randomly divided into control group(treated with mifepristone)and experimental group(treated with ultrasound-guided uterine clearing+mifepristone)according to odd-even method,with 30 cases in each group.The perioperative indexes,complication rate of2 months after operation,return to normal rate of menstruation,the time of HCG descending to normal and the time of complete absorption of accessory mass were compared between the two groups.Results The intraoperative blood loss and hospitalization cost of experimental group were less than those of control group,and the duration of hospitalization and operation was shorter than those of control group(P<0.05).The complication rate of experimental group was significantly lower than that of control group(3.33%VS 26.67%),and the return to normal rate of menstruation was significantly higher than that of control group(96.67%VS 73.33%)(P<0.05).Blood HCG decreased to normal time and complete absorption time ofaccessory mass in experimental group was shorter than that in control group(P<0.05).Conclusion Ultrasound-guided uterine clearing for pregnant women with uterine cicatrix after cesarean section has a remarkable effect and is worthy ofclinical promotion.
