摘要
患者男,68岁,背部广泛性斑丘疹4 d。皮肤科情况:背部可见针尖至米粒大小淡红色斑疹、丘疹及斑丘疹,表面干燥。皮损组织病理示:表皮灶状基底层细胞空泡样液化变性,真皮浅中层血管周围较多上皮样细胞浸润,少许浆细胞、中性粒细胞,可见核尘,部分血管壁受累。免疫组织化学:CD3阳性,CD20阳性。诊断:布鲁菌病。
A 68-year-old male presented with extensive maculopapular on his back for 4 days.Dermatological examination revealed that pale red erythema,papules and maculopapular from pinpoint to rice grain size were seen on the back,and the surface was dry.Skin histopathology showed focal basal layer cells vacuolar liquefaction and degeneration,more epithelioid cells and a few plasma cells and neutrophils infiltrated around blood vessels in the superficial and middle dermis,nuclear dust was visible,and part of the vessel wall was involved.Immunohistochemistry showed CD3 and CD20 were positive.The diagnosis of brucellosis was made.
