外周血SHP-1、Sestrin2水平与缺血性脑卒中患者颈动脉斑块的相关性分析

摘要 目的分析外周血蛋白酪氨酸磷酸酶1(SHP-1)、Sestrin2水平与缺血性脑卒中(CIS)患者颈动脉斑块的相关性。方法选择2018年7月-2021年9月苏州市第九人民医院收治的85例CIS患者为试验组,另选择医院同时间段体检健康人群73例为对照组。分别于入院次日、体检当天检测试验组与对照组血清SHP-1、Sestrin2水平,根据颈动脉斑块是否稳定分为稳定组(59例)与不稳定组(26例),比较对照组与试验组的一般资料,比较稳定组与不稳定组的临床资料,以Logistic回归分析法探讨CIS患者颈动脉斑块不稳定的影响因素,采用Spearman分析血清SHP-1、Sestrin2水平与CIS患者颈动脉斑块不稳定形成的相关性,以受试者工作特征(ROC)曲线下面积(AUC)评估入院时血清Sestrin2、SHP-1水平及二者联合对CIS患者发生颈动脉不稳定斑块的预测价值。结果2组年龄、性别构成、体质量指数(BMI)比较差异无统计学意义(P>0.05),试验组血清SHP-1水平低于对照组(P<0.05),试验组血清Sestrin2水平高于对照组(P<0.05);不稳定组入院时载脂蛋白A1、低密度脂蛋白(LDL-C)、空腹血糖、高敏C反应蛋白(hs-CRP)、尿酸、血清Sestrin2水平高于稳定组(P<0.05),不稳定组入院时血清SHP-1水平低于稳定组(P<0.05);Logistic回归分析结果显示,入院时载脂蛋白A1、尿酸、血清SHP-1、Sestrin2水平均为CIS患者颈动脉斑块不稳定的影响因素(P<0.05);Spearman分析结果显示,血清SHP-1水平与CIS患者颈动脉斑块不稳定形成呈负相关(P<0.05),血清Sestrin2水平与CIS患者颈动脉斑块不稳定形成呈正相关(P<0.05);ROC曲线结果显示,入院时血清Sestrin2、SHP-1水平及二者联合预测CIS患者发生颈动脉不稳定斑块的AUC值分别为0.818、0.824、0.868(P<0.05)。结论血清SHP-1水平与CIS患者颈动脉斑块不稳定形成呈负相关,血清Sestrin2水平与CIS患者颈动脉斑块不稳定形成呈正相关,二者可用于预测CIS患者发生颈动脉不� Objective To analyze the correlation between the levels of src homology region 2 domain-containing phosphatase 1(SHP-1)and Sestrin 2 and carotid artery plaque in patients with cerebral ischemic stroke(CIS).Methods A total of 85 CIS patients admitted to Suzhou Ninth People's Hospital from July 2018 to September 2021 were selected as the experimental group,and 73 healthy people with physical examination at the same time in the hospital were selected as the control group.The serum SHP-1 and Sestrin2 levels of the experimental group and the control group were examination.Depending on whether the carotid artery plaque was stable,they were divided into the stable group(59 cases)and the unstable group(26 cases).General data of the control group and the experimental group were compared.The clinical data of the stable group and the unstable group were compared.Logistic regression analysis was used to explore the infl uencing factors of carotid artery plaque instability in CIS patients.Spearman was used to analyze the correlation between serum SHP-1 and Sestrin2 levels and instability of carotid artery plaque in CIS patients.The area under the receiver operating characteristic(ROC)curve(AUC)was used to evaluate the serum Sestrin2 and SHP-1 levels on admission and the predictive value of the combination of the two for the occurrence of unstable carotid plaque in CIS patients.Results There were no signifi cant differences in age,gender composition,and body mass index(BMI)between 2 groups(P>0.05).The serum SHP-1 level of the experimental group was lower than that of the control group(P<0.05),and the serum Sestrin2 level of the experimental group was higher than that of the control group(P<0.05).The levels of apolipoprotein A1,low density lipoprotein cholesterol(LDL-C),fasting blood glucose,highsensitivity C-reactive protein(hs-CRP),uric acid and serum Sestrin2 on admission in the unstable group were higher than those in the stable group(P<0.05),while the level of serum SHP-1 in the unstable group on admission was lower than t
