摘要
【目的】探讨血清降钙素原(PCT)、C反应蛋白(CRP)、淀粉样蛋白A(SAA)、白细胞介素-6(IL-6)水平与慢性阻塞性肺疾病急性加重期(AECOPD)患者合并呼吸道感染的相关性。【方法】回顾性分析2019年4月至2022年4月本院收治的85例AECOPD患者的临床资料,根据患者是否合并呼吸道感染分为感染组(n=30)和未感染组(n=55)。比较两组患者的一般资料及血清PCT、CRP、SAA、IL-6水平,采用受试者工作特征(ROC)曲线分析PCT、CRP、SAA、IL-6对AECOPD患者合并呼吸道感染的预测价值;采用多因素Logistic回归性分析AECOPD患者合并呼吸道感染的危险因素。【结果】感染组有呼吸衰竭、1年内AECOPD发作次数≥3次的患者占比及PCT、CRP、SAA、IL-6水平显著高于未感染组(P<0.05);PCT、CRP、SAA、IL-6预测AECOPD患者合并呼吸道感染的曲线下面积分别为0.778、0.809、0.865、0.816,呼吸衰竭、1年内AECOPD发作次数≥3次、PCT≥4.795μg/L、CRP≥5.730 mg/L、SAA≥25.310 mg/L、IL-6≥10.535 ng/L是AECOPD患者合并呼吸道感染的危险因素(P<0.05)。【结论】血清PCT、CRP、SAA、IL-6水平与AECOPD患者合并呼吸道感染密切相关,可作为辅助诊断AECOPD合并呼吸道感染的血清指标。
【Objective】To explore the relationship of serum procalcitonin(PCT),C-reactive protein(CRP),serum amyloid A(SAA),and interleukin-6(IL-6)levels with respiratory tract infection in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).【Methods】The clinical data of 85 patients with AECOPD admitted to our hospital from April 2019 to April 2022 were retrospectively analyzed.According to whether the patients were complicated with respiratory tract infection,they were divided into two groups,namely,the infected group(n=30)and the uninfected group(n=55).The general data and serum levels of PCT,CRP,SAA and IL-6 were compared between the two groups.The value of PCT,CRP,SAA and IL-6 in predicting respiratory tract infection in AECOPD patients was analyzed by ROC analysis.The risk factors of AECOPD patients complicated with respiratory tract infection were determined by multivariate logistic regression analysis.【Results】The proportion of patients with respiratory failure and AECOPD attacks≥3 times within one year in the infected group was significantly higher than that in the uninfected group(P<0.05),and the levels of PCT,CRP,SAA and IL-6 in the infected group were significantly higher than those in the uninfected group(P<0.05).By ROC and logistic analysis,the areas under the curve of PCT,CRP,SAA and IL-6 predicting AECOPD patients with respiratory tract infection were 0.778,0.809,0.865 and 0.816,respectively(P<0.05).Respiratory failure,AECOPD attacks≥3 times within 1 year,PCT≥4.795μg/L,CRP≥5.730 mg/L,SAA≥25.310 mg/L and IL-6≥10.535 ng/L were the risk factors of respiratory tract infection in AECOPD patients(P<0.05).【Conclusion】The levels of serum PCT,CRP,SAA and IL-6 are closely related to AECOPD patients with respiratory tract infection,and can be used as serum indicators to assist in the diagnosis of AECOPD patients with respiratory tract infection.
