摘要
目的:对人工耳蜗植入(cochlear implants,CI)治疗的经济学研究进行系统综述,分析研究现状,为我国开展相关研究提供参考。方法:系统检索中国知网、万方和PubMed等中英文数据库以及国际主要卫生技术评估机构的官网,按照纳排标准纳入文献。结果:共纳入23篇研究,覆盖16国家/地区,多数研究针对双侧重度及以上感音神经性聋的儿童,采用Markov模型研究分析单侧和(或)双侧CI的成本效果。多数研究发现单侧CI具有较好的经济性,但是我国相关研究不足。结论:单侧CI具有较好的成本效果,但结果受国家经济发展、卫生体系、卫生资源配置等因素的影响。建议我国应从全社会角度出发,分别围绕单侧CI和双侧CI开展全人群终身健康、教育和就业全面获益的经济学评价研究。
Objective:To review the economics evaluations of cochlear implant(CI)treatment worldwide,so as to provide references to enlighten further research in China.Methods:The Chinese and English databases such as CNKI,Wanfang and PubMed,and the official websites of major international health technology assessment institutions were systematically searched.The literature was included according to the inclusion and ranking criteria.Results:Twenty-three studies covered 16 countries/regions,most of which targeted children with bilateral severe sensorineural hearing loss and above.The Markov model was most commonly used to analyze the cost-effectiveness of unilateral and(or)bilateral CI.Most studies had found that one-sided CI was cost-effective,but studies on Chinese population was insufficient.Conclusion:Unilateral CI shows cost-effective,but it is affected by factors such as economic development,health system,and health resource allocation in different countries.Further economic evaluation and social benefit research china should be conducted in line with national conditions,comapring differrent CI strategies,targeting the whole population,convering the life-cycle and overall benefits from social perspective.
