摘要
目的了解2015—2021年阿坝州恶性肿瘤发病情况,为阿坝州的肿瘤防治提出有针对性科学防控依据。方法从“中国肿瘤登记平台”中导出阿坝州2015—2021年恶性肿瘤病例,采用SAS 9.4计算分性别、年龄和年度的粗发病率和标化发病率,发病率随年龄的变化趋势采用趋势卡方检验,用年度变化百分比(APC)反映发病率随年度的变化情况,差异比较采用t检验,检验水准α=0.05。结果2015—2021年阿坝州死亡发病比为0.39、形态学诊断确诊病例比例为39.7%、仅有死亡医学证明书病例比例为6.61%,粗发病率和标化发病率分别为87.21/10万、71.50/10万,发病率随年度变化的百分比(APC)差异无统计学意义(粗发病率t=2.42、P=0.060,标化发病率t=2.11、P=0.088),发病前5位的恶性肿瘤分别为:肺癌、胃癌、肝癌、食管癌和结直肠癌。结论阿坝州肿瘤发病数据M/I、MV、DCO等质控指标显示数据上报质量较差、发病率较低,需要结合实际,提高肿瘤登记随访数据的质量;针对高发癌种和高危人群有针对性地进行健康教育和癌症早期筛查。
Objective To understand the incidence of malignant tumors in Aba Tibetan and Qiang Autonomous Prefecture in 2015 to 2021 so as to put forward targeted suggestions for tumor prevention and treatment in this area.Methods Malignant tumor cases in Aba Prefecture in 2015 to 2021 were extracted from China Tumor Registration Platform.The SAS 9.4 was used to calculate crude and standardized incidence adjusted by sex,age and year.The trend of incidence with age was analyzed by chi-square trend tests.The annual percentage change(APC)was calculated to reflect the change in incidence from year to year.Results From 2015 to 2021,the mortality incidence ratio was 0.39,the proportion of cases diagnosed by morphological diagnosis was 39.7%,and the proportion of cases with only death certificates was 6.61%.The incidence and standardized incidence was 87.21/105 and 71.50/105,respectively.The percentage change in incidence from year to year (APC) was not statistically significant (crude incidence, t=2.42, P=0.060;standardizedincidence, t=2.11, P=0.088) . The top 5 malignant tumors with high incidence were lung,gastric, liver, esophageal and colorectal cancers. Conclusions Quality control indicators of tumorincidence data in Aba prefecture showed poor quality. It is necessary to improve the quality oftumor registration and follow-up. Targeted health education and early cancer screening for high-riskgroups should be undertaken.
