摘要
目的探讨奥曲肽联合经鼻肠梗阻导管置入术对晚期恶性小肠梗阻的应用效果。方法选取2018年1月至2022年6月北京航天总医院收治的102例晚期恶性小肠梗阻患者为研究对象,采用随机数字表法将入选患者分为观察组和对照组,每组各51例。对照组患者予常规对症处理与经鼻肠梗阻导管置入术,观察组患者在此基础上联用奥曲肽治疗。比较两组患者的临床症状改善状况、临床疗效及治疗前后血清C反应蛋白(C-reactive protein,CRP)、分泌型免疫球蛋白A(secretory immunoglobulin A,SIgA)、丙二醛(malondialdehyde,MDA)水平。结果观察组患者腹痛腹胀消失时间、恶心呕吐消失时间、恢复排气时间均显著早于对照组(均P<0.05)。两组患者临床总有效率比较差异无统计学意义(P>0.05)。与组内治疗前相比,两组患者治疗后血清CRP、MDA水平均显著降低(均P<0.05),血SIgA水平均显著升高(均P<0.05)。与同期对照组相比,观察组患者治疗后血清CRP、MDA水平均显著降低(均P<0.05),血SIgA水平显著升高(P<0.05)。结论奥曲肽联合经鼻肠梗阻导管置入术能明显抑制晚期恶性肠梗阻患者的肠道炎症反应和氧化应激反应,缓解临床症状,促进早日恢复肛门排气。
Objective To investigate the effect of octreotide combined with transnasal intestinal obstruction catheter implantation in the treatment of advanced malignant intestinal obstruction.Method One hundred and two patients with advanced malignant intestinal obstruction admitted to Beijing Aerospace General Hospital from January 2018 to June 2022 were prospectively selected as the study subjects and randomly divided into observation group and control group,with 51 cases in each group.Patients in control group were treated with conventional symptomatic treatment and transnasal intestinal obstruction catheter implantation,while those in observation group were additionally treated with octreotide.The improvement of clinical symptoms,clinical efficacy and serum levels of C-reactive protein(CRP),secretory immunoglobulin A(SIgA)and malondialdehyde(MDA)before and after treatment were compared between the two groups.Result The disappearance time of abdominal pain,abdominal distension,nausea and vomiting and the time of recovering exhaust in observation group were significantly earlier than those in control group(all P<0.05).There was no significant difference in the total clinical effective rate between the two groups(P>0.05).Compared with intra-group before treatment,the serum levels of CRP and MDA after treatment in the two groups were significantly decreased(all P<0.05),and the serum levels of SIgA were significantly increased(all P<0.05).Compared with control group at the same time,the serum levels of CRP and MDA after treatment in observation group were significantly decreased(all P<0.05),and the serum level of SIgA was significantly increased(P<0.05).Conclusion Octreotide combined with transnasal intestinal obstruction catheter implantation can significantly inhibit intestinal inflammatory reaction and oxidative stress in patients with advanced malignant intestinal obstruction,relieve clinical symptoms,and promote the early recovery of anal exhaust.
