摘要
目的全面了解全国跨区域专科联盟建设和发展情况,探讨专科联盟建设和发展过程中面临的困境,推进其可持续发展。方法数据来自国家卫生健康委卫生发展研究中心开展的2022年全国专科联盟绩效评估工作。采用定量和定性相结合的方法,定量和定性资料分别采用描述性统计方法和主题框架分析法进行分析。结果各省(区、市)以各种形式陆续探索和尝试开展专科联盟建设工作,各级各类医疗机构参与积极性较高,牵头医院在人才、技术等方面发挥了较好的带动辐射作用,联盟内上下转诊逐步畅通、协同网络作用有所显现。但也发现:跨区域专科联盟建设存在层次和结构需进一步优化、运行机制有待进一步健全、监督考核与约束机制有待进一步加强、医疗服务同质化水平有待进一步提高、各成员单位信息化水平存在差异等问题和挑战。结论需政府引导做好资源规划,重视监督考核评价制度的构建,强化人才培养和专科诊疗能力以提高专科联盟同质化水平,发挥信息化支撑作用,实现资源互联互通。
Objective To gain a comprehensive understanding of the construction and development of national cross-regional specialty alliances,explore the dilemmas faced in the construction and development of specialty alliances,and promote their standardized development.Methods A combination of quantitative and qualitative methods was used.Data were obtained from the 2022 national specialty alliance performance assessment.Quantitative and qualitative information were analyzed using descriptive statistical methods and thematic framework analysis.Results China has explored and tried to carry out specialty alliances in various forms one after another,medical institutions at all levels and types have been more active in participating,the leading hospitals have played a better role in driving radiation in terms of talent and technology,the upward and downward referrals within the alliance have been gradually smooth,and the synergistic network has been revealed.However,problems and challenges such as the need for further optimization of the hierarchy and structure,the need for further improvement of the operation mechanism,the need for optimization of the supervision,assessment and restraint mechanism,the need for further improvement of the level of homogenization of medical services,and the differences in the level of informationization of each member unit were also found.Conclusion It is necessary for the government to guide the planning of resources,pay attention to the construction of supervision and assessment and evaluation system,strengthen the training of talents and the ability of specialty treatment in each unit to improve the level of homogenization within the alliance,and play the role of information technology support to realize the interconnection of resources.
