摘要
目的回顾性分析腹腔镜手术与传统开腹手术治疗低位直肠癌的临床资料,比较两种术式的优劣势,以期对选择治疗措施提供理论依据。方法选取经手术治疗的低位直肠癌患者186例作为研究对象,根据手术治疗方式不同将其分为两组,其中腹腔镜组89例,传统开腹组97例。收集两组研究对象的病历资料,主要包括手术时间、术中出血量、住院时间、术后排气时间、吻合口瘘及吻合口出血发生率、非计划再手术率、左结肠动脉保留率、副损伤发生率、复发及转移发生率。结果腹腔镜组的术中出血量、术后排气时间、住院时间、左结肠动脉保留率低于传统开腹组(P<0.05),手术时间高于对照组(P<0.05)。两组的吻合口瘘或出血的发生率、非计划再手术率比较,差异无统计学意义(P>0.05)。并发症及远期随访比较,差异无统计学意义(P>0.05)。结论与传统开腹手术相比,腹腔镜手术的术后并发症与远期疗效无明显差异,但是手术创伤更小,更有利于患者术后恢复,值得临床推广与运用。
Objective To retrospectively analyze the clinical data of laparoscopic surgery and traditional laparotomy in the treatment of low rectal cancer,and compare the advantages and disadvantages of laparoscopic surgery and traditional surgery in order to provide theoretical basis for the selection of treatment measures.Methods A total of 186 patients with low rectal cancer treated by surgery were selected as the study subjects,on the basis of different surgical treatment methods,which were divided into two groups,with 89 cases in the laparoscopy group and 97 cases in the traditional laparotomy group.The medical records of the two groups of subjects were collected,mainly including the operation time,intraoperative bleeding,hospital stay,postoperative exhaust time,incidence of anastomotic leakage and anastomotic bleeding,unplanned reoperation rate,left colon artery preservation rate,injury rate,recurrence and metastasis rate.Results The intraoperative bleeding volume,postoperative exhaust time,hospital stay and left colon artery retention rate in the laparoscopic group were lower than those in the traditional laparotomy group(P<0.05),and the operation time in the laparoscopic group was longer than that in the traditional laparotomy group(P<0.05).There was no statistical difference between the two groups in the incidence of anastomotic leakage or the incidence of bleeding and the rate of unplanned reoperation(P<0.05).There was no significant difference in complications and long-term follow-up(P<0.05).Conclusion Compared with traditional laparotomy,there is no significant difference in the postoperative complications and long-term effects of laparoscopic surgery,but laparoscopic surgery for low rectal cancer has less trauma and it is more conducive to postoperative recovery of patients,which is worthy of clinical promotion and application.
