摘要
目的探讨T细胞亚群与迁延性细菌性支气管炎(PBB)气道损害的相关性。方法选取2019年6月至2022年1月来成都市龙泉驿区妇幼保健院就诊或体检的112例PBB患儿(PBB组)、112例非PBB患儿(非PBB组)及112例健康体检儿童(对照组)作为研究对象。比较各组及PBB组不同气道损害程度儿童CD3^(+)T细胞、CD4^(+)T细胞、CD8^(+)T细胞、CD4^(+)/CD8^(+)水平,以及不同T细胞亚群水平儿童气道损害的危险度。应用Spearman分析T细胞亚群与PBB气道损害程度关系,采用多因素Logistic回归方程分析PBB气道损害的相关影响因素,采用受试者工作特征(ROC)曲线及曲线下面积(AUC)分析T细胞亚群评估PBB气道损害的价值。结果3组CD3^(+)T细胞、CD4^(+)T细胞、CD8^(+)T细胞、CD4^(+)/CD8^(+)比较,差异均有统计学意义(P<0.05)。与无气道损害者比较,轻度、中度、重度气道损害患儿CD3^(+)T细胞、CD8^(+)T细胞呈降低趋势,CD4^(+)T细胞、CD4^(+)/CD8^(+)呈升高趋势,两两比较差异均有统计学意义(P<0.05);CD3^(+)T细胞、CD4^(+)T细胞、CD8^(+)T细胞、CD4^(+)/CD8^(+)异常者发生气道损害的风险高于正常者(P<0.05)。CD3^(+)T细胞、CD8^(+)T细胞与PBB气道损害程度呈负相关(P<0.05),CD4^(+)T细胞、CD4^(+)/CD8^(+)与PBB气道损害程度呈正相关(P<0.05)。多因素Logistic回归分析显示,CD3^(+)T细胞、CD4^(+)T细胞、CD8^(+)T细胞、CD4^(+)/CD8^(+)均与PBB气道损害显著相关(P<0.05);CD3^(+)T细胞、CD4^(+)T细胞、CD8^(+)T细胞、CD4^(+)/CD8^(+)评估PBB气道损害的AUC依次为0.783、0.751、0.764、0.804,各指标联合评估的AUC为0.892(P<0.05)。结论PBB患儿T淋巴细胞亚群紊乱,这与气道损害及其严重程度密切相关,4项指标联合检测有望为PBB患儿气道损害的评估提供一种非侵入、便捷性的手段,并为临床防治PBB及气道损害提供了一个新思路。
Objective To investigate the correlation between T-cell subsets and airway damage in protracted bacterial bronchitis(PBB).Methods From June 2019 to January 2022,112 children with PBB(PBB group),112 children without PBB(non-PBB group)and 112 healthy children with physical examination(control group)who visited Longquanyi District of Maternal and Child Health Care Hospital were selected as subjects.The levels of CD3^(+)T cells,CD4^(+)T cells,CD8^(+)T cells,CD4^(+)/CD8^(+)in children with different airway damage levels in each group were compared,as well as the risk of airway damage in children with different levels of T cell subsets.Spearman was used to analyze the relationship between T cell subsets and the degree of airway damage of PBB,multivariate Logistic regression equation was used to analyze the related influencing factors of airway damage of PBB,and receiver operating characteristic(ROC)curve and area under ROC curve(AUC)were used to analyze the value of T cell subsets in evaluating airway damage of PBB.Results There were statistically significant differences in CD3^(+)T cells,CD4^(+)T cells,CD8^(+)T cells and CD4^(+)/CD8^(+)in three groups(P<0.05).Compared with those without airway damage,CD3^(+)T cells and CD8^(+)T cells were decreased in mild,moderate and severe children with airway damage,while CD4^(+)T cells and CD4^(+)/CD8^(+)were increased,and the pairwise differences were statistically significant(P<0.05).Patients with abnormal CD3^(+)T cells,CD4^(+)T cells,CD8^(+)T cells and CD4^(+)/CD8^(+)cells had a higher risk of airway damage than normal patients(P<0.05).CD3^(+)T cells and CD8^(+)T cells negatively correlated with airway damage of PBB(P<0.05),while CD4^(+)T cells and CD4^(+)/CD8^(+)positively correlated with airway damage of PBB(P<0.05).Multivariate Logistic regression analysis showed that CD3^(+)T cells,CD4^(+)T cells,CD8^(+)T cells,CD4^(+)/CD8^(+)significantly correlated with airway damage of PBB(P<0.05).The AUCs of CD3^(+)T cells,CD4^(+)T cells,CD8^(+)T cells,CD4^(+)/CD8^(+)in the assessment
