摘要
目的:观察创伤性颈部脊髓损伤气管切开患者进行阈值压力负荷呼吸训练对患者肺功能的影响。方法:将50例创伤性颈部脊髓损伤气管切开患者随机分为对照组和实验组(每组各25例),对照组给予常规的呼吸功能训练;实验组在对照组的基础上同时给予阈值压力负荷呼吸功能训练;分别于训练前及训练4周后评估患者肺功能[用力肺活量(forced vital capacity,FVC)、第一秒用力呼气容积(forced expiratory volume in one second,FEV1)]及最大通气量(maximal ventilator volume,MVV)的变化及治疗后拔除气切套管的时间及拔管率。结果:与训练前比较,两组受试者训练4周后的FVC、FVC%pred、FEVl、FEV1%pred、MVV及MVV%pred均有明显提高(P<0.01);训练4周后,实验组FVC、FVC%pred、FEV1、FEV1%pred、MVV及MVV%pred均高于对照组(P<0.01);实验组患者拔管时间明显缩短,拔管率显著提高,差异具有统计学意义(P<0.05)。结论:创伤性颈部脊髓损伤气管患者接受阈值压力负荷呼吸功能训练能改善患者肺功能,同时能缩短拔除气切套管时间、提高拔管率。
Objective:To observe the effectiveness of threshold inspiratory muscle training on lung function in patients with traumatic cervical spinal cord injury and undergoing tracheotomy.Methods:A total of 50 patients with traumatic cervical spinal cord injury and undergoing tracheotomy were randomly assigned to the control and experimental groups(n=25 for each group).Patients in the control group were regularly treated with physical training and respiratory training,once a day.Patients in the experimental group were given additional hyperbaric oxygen theraty,once a day.All subjects were received the training each day,five times a week for four weeks.All patients were examined lung function which included forced vital capacity(FVC),forced expiratory volume in one second(FEV1)and maximal ventilator volume(MVV)and the time of decannulation and the rat of decannulation after treatment.Results:Compared with before the training,FVC,FVC%pred,FEVl,FEV1%pred,MVV and MVV%pred in two groups obviously improved(P<0.01).After three-week training,comparing with the control group,FVC,FVC%pred,FEVl,FEV1%pred,MVV and MVV%pred in experimental group were significantly high(P<0.01).Comparing with the control group,The time of decannulation was less and the rat of decannulation were higher in the treatment group(P<0.05).Conclusions:Threshold inspiratory muscle training can significantly improve on lung function in patients with traumatic cervical spinal cord injury and undergoing tracheotomy,and the less time of decannulation and the higher rat of decannulation.
