摘要
目的:检测急性心肌梗死(acute myocardial infarction,AMI)患者外周血Maresin-1表达水平并探讨其与AMI患者预后的关系。方法:选取我院2016年9月至2018年10月,收治的84例AMI患者为研究对象,同期在我院体检的84例健康体检者作为对照组,检测两组患者外周血Maresin-1的表达水平,分析Maresin-1表达水平与AMI患者发生主要不良心血管事件(major adverse cardiovascular events,MACE)的相关性。结果:AMI组患者外周血Maresin-1表达水平显著低于对照组的(P<0.05);Maresin-1低表达组患者的血清CK-MB峰值、cTnI峰值、BNP和Gensini评分显著高于Maresin-1高表达组,而LVEF显著低于Maresin-1高表达组(P<0.05);多因素Cox比例风险模型结果显示血清CK-MB峰值、cTnI峰值和BNP是AMI组患者Maresin-1表达水平的影响因素(P<0.05);Maresin-1低表达组患者MACE的累积发生率显著高于Maresin-1高表达组(P<0.05);外周血Maresin-1水平具有预测AMI患者MACE的价值(ROC曲线下面积为0.876)。当Maresin-1浓度为95.848 ng/L时的敏感性为92.6%,特异性80.6%。结论:AMI患者外周Maresin-1的表达水平显著降低,且Maresin-1低表达水平于AMI患者的不良预后相关。
Objective:To observe the expression of Maresin-1 in peripheral blood of patients with acute myocardial infarction(AMI)and its relationship with prognosis,Methods:A total of 84 patients with AMI were selected and 84 healthy physical examiners who received physical examination in our hospital in the same period were selected as control group.The expression level of Maresin-1 in peripheral blood of the two groups were detected,and the correlation between the expression level of Maresin-1 and major adverse cardiovascular events(MACE)in patients with AMI were analyzed.Results:The expression level of Maresin-1 in peripheral blood of patients with AMI was significantly lower than that of the control group(P<0.05).The serum CKMB peak,cTnI peak,BNP and Gensini scores in patients with low expression of Maresin-1 were significantly higher than those in patients with high expression of Maresin-1,while LVEF was significantly lower in patients with high expression of Maresin-1(P<0.05).The results of Cox proportional hazard model showed that serum CK-MB peak,cTnI peak and BNP were the influencing factors of Maresin-1 expression level in patients with AMI(P<0.05).The cumulative incidence of MACE in patients with low expression of Maresin-1 was significantly higher than that in patients with high expression of Maresin-1(P<0.05).The level of peripheral blood Maresin-1 has the value of predicting MACE in patients with AMI(the area under the ROC curve is 0.876).When the concentration of Maresin-1 was 95.848 pg/mL,the sensitivity was 92.6%and the specificity was 80.6%.Conclusions:The expression level of peripheral Maresin-1 in AMI patients was significantly reduced,and the low expression level of Maresin-1 was related to the poor prognosis of AMI patients.
