摘要
目的:分析关节镜辅助复位内固定术(ARIF)治疗胫骨平台骨折的效果。方法:选择2017年8月—2021年3月中国人民解放军联勤保障部队第九〇〇医院仓山院区收治的153例胫骨平台骨折患者,按照随机数表法分为两组,ORIF组(n=76)接受切开复位内固定(ORIF)治疗,ARIF组(n=77)接受关节镜辅助复位内固定术(ARIF)治疗。观察两组手术一般情况、炎症因子水平、膝关节功能及并发症发生率。结果:ARIF组手术时间、术后完全负重时间、住院时间、术中出血量、切口长度均优于ORIF组,差异有统计学意义(P<0.05)。术后24 h,ARIF组各项炎症因子水平均低于ORIF组,差异有统计学意义(P<0.05)。对所有患者进行为期1年的随访,失访9例,两组共计纳入144例,各72例。随访结束后,ARIF组膝关节功能各评分均高于ORIF组,差异有统计学意义(P<0.05)。ARIF组并发症发生率为4.17%,低于ORIF组的13.89%,差异有统计学意义(P<0.05)。结论:相比较于切开复位内术,运用ARIF术治疗胫骨平台骨折患者可更好地改善手术一般情况,减轻炎症反应,加快膝关节功能恢复,且并发症发生率更低。
Objective:To analyze the effect of arthroscopic assisted reduction and internal fixation(ARIF)on tibial plateau fractures.Method:A total of 153 patients with tibial plateau fractures treated in Cangshan Hospital of the 900th Hospital of the Joint Logistics Support Force of the Chinese People's Liberation Army from August 2017 to March 2021 were divided into two groups according to random number table.The ORIF group(n=76)was treated with open reduction and internal fixation(ORIF),and ARIF group(n=77)was treated with ARIF.The general situation of operation,inflammatory factors,knee joint function and the incidence of complications were observed in the two groups.Result:The operation time,total weight bearing time after operation,hospital stay,intraoperative bleeding volume and incision length in ARIF group were significantly better than those in ORIF group,the differences were statistically significant(P<0.05).At 24 hours after operation,the levels of various inflammatory factors in ARIF group were lower than those in ORIF group,and the differences were statistically significant(P<0.05).All patients were followed up for 1 year,9 cases were lost,144 cases were included,72 cases in each group.After the follow-up,the scores of knee joint function in ARIF group were higher than those in ORIF group,and the differences were statistically significant(P<0.05).The incidence of complications in ARIF group was 4.17%,which was lower than 13.89%in ORIF group,and the difference was statistically significant(P<0.05).Conclusion:Compared with open reduction and internal fixation,arthroscopic assisted reduction and internal fixation can better improve the general feeling of surgery,reduce the inflammatory reaction,accelerate the recovery of knee joint function,and the incidence of complications is lower.
