超声造影联合肿瘤标志物监测评估肝癌射频消融术后疗效与消融病灶的价值

The value of contrast-enhanced ultrasound combined with tumor marker in monitoring and assessing the curative effect after radiofrequency ablation for liver cancer and the condition of ablated lesion

摘要 目的:研究超声造影联合肿瘤标志物监测评估肝癌射频消融术后疗效与消融病灶。方法:选择117例在医院接受肝癌射频消融术的患者,依据预后情况将其分为预后良好组(59例,69个病灶)和预后不良组(58例,65个病灶)。使用超声造影联合肿瘤标志物监测评估肝癌射频术后疗效,比较两组患者的一般临床资料,分析手术前后甲胎蛋白(AFP)、糖类抗原(CA19-9)及癌胚抗原(CEA)的水平,研究术后肿瘤完全消除率的情况,评估手术前后的超声造影情况、超声造影监测及超声造影联合肿瘤标志物联合监测对肝癌的诊断效能。结果:预后良好组术后6个月的AFP、CA19-9及CEA显著低于预后不良组,差异有统计学意义(t=14.712,t=53.579,t=11.611;P<0.05);预后良好组术后肿瘤完全消融率为88.40%,显著高于预后不良组的64.62%,差异有统计学意义(χ^(2)=10.653,P<0.05);预后良好组术后6个月的射频消融灶截面积显著高于预后不良组,差异有统计学意义(t=16.468,P<0.05);超声造影与肿瘤标志物联合监测的特异度为95.67%、灵敏度为87.65%,显著高于且优于超声造影监测(特异度84.15%、灵敏度71.44%)。结论:超声造影联合肿瘤标志物监测可有效提高射频消融术的疗效,提高诊断的准确性和术后肿瘤完全消融率,减轻患者的痛苦,为临床治疗肝癌提供评估依据。 Objective:To explore the value of contrast-enhanced ultrasound combined with tumor marker in monitoring and assessing the curative effect after radiofrequency ablation for liver cancer and the condition of ablated lesion.Methods:A total of 117 patients who underwent radiofrequency ablation for liver cancer in hospital were selected as the subjects of this experiment.According to the prognosis,they were divided into favorable prognosis group(group A,59 cases)and unfavorable prognosis group(group B,58 cases).Contrast-enhanced ultrasound combined with tumor marker was used to monitor and assess the curative effect post radiofrequency ablation for liver cancer.The general clinical data of the two groups were compared,and the levels of alpha fetoprotein(AFP),carbohydrate antigen 19-9(CA19-9)and carcinoembryonic antigen(CEA)before and after surgery were analyzed.The complete elimination rate of tumor was researched after surgery.Additionally,the condition of contrast-enhanced ultrasound,and the diagnostic efficiencies of contrast-enhanced ultrasound and contrast-enhanced ultrasound combined with tumor marker in monitoring liver cancer were assessed before and after surgery.Results:The AFP,CA19-9 and CEA at the 6th month after surgery in group A were significantly lower than those in group B(t=14.712,t=53.579,t=11.611,P<0.05),respectively.The complete ablation rate of tumor after surgery in group A was 88.40%,which was significantly higher than that in group B(64.62%)(χ^(2)=10.653,P<0.05).The sectional area of the lesion of radiofrequency ablation at the 6th months after surgery in group A was significantly higher than that in group B(t=16.468,P<0.05).The specificity and sensitivity of contrast-enhanced ultrasound combined with tumor marker were 95.67%and 87.65%in monitoring liver cancer,which were significantly higher than those(84.15%and 71.44%)of contrast-enhanced ultrasound.Conclusion:The monitoring of contrast-enhanced ultrasound combined with tumor marker can effectively improve the curative effect of radiofreque
