摘要
目的 探究罗哌卡因复合氢吗啡酮用于硬膜外分娩镇痛对产程的影响。方法 选取2021年10月至2022年1月南京医科大学附属妇产医院收治的选择硬膜外分娩镇痛的280例产妇,采用随机数字表法分为罗哌卡因+氢吗啡酮组(RH组)和单纯氢吗啡酮组(H组)。观察两组产妇产程时长、改良Bromage运动评分、VAS镇痛评分、分娩方式、氢吗啡酮消耗量、新生儿Apgar评分、脐动脉血气分析、不良反应等指标。结果 最终纳入产妇共245例,RH组121例,H组124例。RH组第一产程平均时间为536.9 min,第二产程为52.3 min;H组第一产程平均时间为516.9 min,第二产程为50.1 min。两组的产程时间、分娩方式、新生儿结局及VAS镇痛评分差异均无统计学意义(P>0.05);与H组相比,RH组的恶心、呕吐及头晕等不良反应发生率较低(P<0.05),氢吗啡酮消耗量较低(P<0.05)。结论 罗哌卡因复合氢吗啡酮用于硬膜外分娩镇痛不影响产程持续时间、运动阻滞程度、分娩方式及新生儿结局,镇痛效果良好,不良反应发生率低。
Objective e To investigate the effects of ropivacaine combined with hydromorphone for epidural labor analgesia on labor process.Methods From October 2021 to January 2022,280 pregnant women admitted to the Women's Hospital Affliated to Nanjing Medical University undergoing epidural analgesia were selected.According to random number table method,patients were divided into group RH(ropivacaine+hydromorphone)and group H(hydromorphone).The duration of labor,modified Bromage motor score,VAS analgesia score,delivery method,consumption of hydromorphone,Apgar score of newborns,umbilical artery blood gas analysis,adverse reactions,and other indicators were observed in both groups.ResultsA total of 245 participants completed the study,with 121 in group RH and 124 women in group H,respectively.In the RH group,the average time of the first stage of labor was 536.9 min,and the second stage of labor was 52.3 min;the average time of the first stage of labor in group H was 516.9 minutes,and the second stage of labor was 50.1 minutes.There was no statistically significant difference in the duration of labor,mode of delivery,neonatal outcome,and VAS analgesia score between the two groups(P>0.05).Compared with group H,the incidence of adverse reactions such as nausea,vomiting,and dizziness in the RH group was lower,and the consumption of hydromorphone was lower(P<0.05).Conclusion The use of ropivacaine combined with hydromorphone for epidural labor analgesia does not affect the duration of labor,degree of motor block,delivery mode,and neonatal outcome.The analgesic effect is good,and the incidence of adverse reactions is low.
