摘要
目的探讨C1q肿瘤坏死因子相关蛋白(CTRP)3在老年2型糖尿病(T2DM)合并骨质疏松(OP)患者血清中表达及其与骨折风险的关系。方法选取老年T2DM合并OP患者174例作为T2DM合并OP组,根据患者在随访期间是否出现新发骨折将其分为骨折组和无骨折组。选取同期单纯老年T2DM患者50例作为T2DM组,另选取同期体检的健康老年人50例作为对照组。采用酶联免疫试剂盒检测血清CTRP3表达。分析各组间血清CTRP3表达差异、老年T2DM合并OP患者血清CTRP3与其他指标的相关性及血清CTRP3对老年T2DM合并OP患者骨折风险的预测价值。结果T2DM合并OP组、T2DM组、对照组血清CTRP3水平逐渐升高,两两比较差异均有统计学意义(P<0.05);骨折组糖尿病病程、胰岛素抵抗指数(HOMA-IR)均显著高于无骨折组,血清CTRP3水平显著低于无骨折组(P<0.05);老年T2DM合并OP患者血清CTRP3与体质量指数、糖尿病病程、空腹血糖(FBG)、HOMA-IR均呈显著负相关(P<0.05);血清CTRP3对老年T2DM合并OP患者骨折风险的预测价值较高,曲线下面积为0.862(95%CI:0.807~0.917),灵敏度、特异度和约登指数分别为68.57%、94.24%、0.628。结论CTRP3在老年T2DM合并OP患者血清中表达下调,且其表达水平与骨折风险密切相关。
Objective To investigate the effect of C1q tumor necrosis factor-related protein(CTRP)3 in serum of elderly patients with type 2 diabetes mellitus(T2DM)and osteoporosis(OP)expression and its relationship with fracture risk.Methods 174 elderly patients with T2DM and OP were selected as the T2DM and OP group,according to whether the patients had new fractures during the follow-up period,they were divided into fracture group and non-fracture group.50 elderly patients with T2DM alone during the same period were selected as the T2DM group,and 50 healthy elderly patients who received physical examination during the same period were selected as the control group.The expression of serum CTRP3 was detected by enzyme-linked immunosorbent assay kit.The differences of serum CTRP3 expression among different groups were analyzed,the correlation between serum CTRP3 and other indexes in elderly T2DM patients with OP were analyzed,the predictive value of serum CTRP3 on fracture risk in elderly T2DM patients with OP were analyzed.Results Serum CTRP3 levels of T2DM and OP group,T2DM group and control group gradually increased,and the differences between any two groups were statistically significant(P<0.05);the course of diabetes and homeostasis model assessment of insulin resistance index(HOMA-IR)in the fracture group were significantly higher than those in the non-fracture group,and the level of serum CTRP3 was significantly lower than that in the non-fracture group(P<0.05);serum CTRP3 in elderly patients with T2DM and OP was negatively correlated with body mass index,diabetes course,fasting blood glucose(FBG)and HOMA-IR(P<0.05);serum CTRP3 had high predictive value for fracture risk in elderly patients with T2DM and OP,the area under the curve was 0.862(95%CI:0.807~0.917).The sensitivity,specificity and yoden index were 68.57%,94.24%and 0.628 respectively.Conclusions The expression of CTRP3 is down-regulated in the serum of elderly T2DM patients with OP,and its expression level is closely related to the risk of fracture.
