阿卡波糖联合双歧杆菌三联活菌制剂治疗非酒精性脂肪性肝病的疗效 被引量:1

Efficacy of acarbose combined with bifidobacterium triple viable preparation in treating non-alcoholic fatty liver disease

摘要 目的探讨阿卡波糖联合双歧杆菌三联活菌制剂治疗非酒精性脂肪性肝病(NAFLD)的疗效。方法将64例NAFLD患者随机分为对照组和观察组,每组32例。2组均给予非药物康复措施和阿卡波糖(50 mg,口服,每日3次),观察组加用双歧杆菌三联活菌制剂(420 mg,口服,每日2次),疗程均为3个月。比较2组治疗前后肝功能、胰岛素抵抗指数、血脂、内脏脂肪含量、肝/脾CT值、血清炎症因子、脂肪因子、肝因子水平等,并观察不良反应发生情况。结果对照组和观察组分别完成31例和32例,治疗前2组各项指标比较均无显著差异(P>0.05)。治疗后,观察组患者的腰围、内脏脂肪面积、体重指数、丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)、谷氨酰转移酶(GGT)、血清总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、稳态模型评估-胰岛素抵抗指数(HOMA-IR)、超敏C反应蛋白(hs-CRP)、白细胞介素(IL)-6、肿瘤坏死因子(TNF)-α、核因子(NF)-κB、视黄醇结合蛋白(RBP)4、抵抗素(resistin)、脂肪因子chemerin、促代谢因子betatrophin及成纤维细胞生长因子(FGF)21水平均低于对照组,差异有显著意义(P<0.05);胰岛素样生长因子(IGF)-1、高密度脂蛋白胆固醇(HDL-C)、脂联素水平及肝/脾CT值高于对照组,有显著差异(P<0.05)。2组均未发生明显不良反应。结论阿卡波糖联合双歧杆菌三联活菌制剂治疗NAFLD可恢复患者肝功能,提高临床疗效,且安全。 AIM To investigate the efficacy of acarbose combined with bifidobacterium triple viable preparation in the treatment of non-alcoholic fatty liver disease(NAFLD).METHODS Sixty-four outpatients with NAFLD were randomly divided into control group(n=32)and observation group(n=32).The patients in the two groups were given non-drug rehabilitation measures according to the guidelines and acarbose(50 mg,orally,3 times every day),while the patients in the observation group were added with bifidobacterium triple viable preparation(420 mg,orally,twice a day),continuously for 3 months.The levels of liver enzymes,insulin resistance,blood lipid,visceral fat content,serum inflammatory factor level,serum fat factor level,serum liver factor level,ratio of liver to spleen CT value and FIB-4 index were compared between the two groups before and after treatment.Meanwhile,the adverse reactions of patients were observed.RESULTS There were 31 patients and 32 patients completed the trial in the control group and the observation group,respectively.There was no significant difference in the indexes between the two groups before treatment(P>0.05).After treatment,the waist circumference,visceral fat area,body mass index(BMI),the levels of alanine aminotransferase(ALT),aspartate aminotransferase(AST),γ-glutamyl transferase(GGT),serum total cholesterol(TC),triglycerides(TG),low density lipoprotein cholesterol(LDL-C),homeostasis model of assessment-insulin resistance(HOMA-IR),highsensitivity C-reactive protein(hs-CRP),interleukin(IL)-6,tumor necrosis factor(TNF)-α,nuclear factor(NF)-κB,retinol binding protein 4(RBP4),resistin,chemerin,betatrophin and fibroblast growth factor 21(FGF21)in the observation group were significantly lower than those in the control group(P<0.05).Compared with the control group,the levels of insulin-like growth factor(IGF)-1,high density lipoprotein cholesterol(HDL-C),adiponectin and the ratio of liver to spleen CT in the observation group were significantly higher than those in the control group(P<0.05).No obvious
