摘要
目的 探析张越中药治疗肺癌的用药规律。方法 收集2019年3月-2021年12月期间吉林省名中医张越于吉林省肿瘤医院门诊治疗肺癌患者的中药处方,整理录入古今医案云平台,对处方药物的用药频次、气味归经、关联、聚类关系进行统计分析。结果 纳入病历248例,男女患者比例相当,年龄集中在50~80岁,其中最小年龄26岁,最大年龄90岁。用药频次≥80的有16味药,浙贝、黄芪、白术、佛手等药物为高频药物。药物多苦、甘、辛味,四气以平药居多,主要归肺、脾胃、肝经。治法主要是清热解毒、燥湿化痰、生津养血、补气升阳等。结论 张越中药治疗肺癌强调“治人而非治病”的整体辨证思路,扶正祛邪兼顾,寒热平调,扶正重点在于补气健脾,疏肝益肺;祛邪重点在于清热解毒,化痰软坚。
Objective To explore the laws of using Chinese medicines in the treatment of lung cancer by Professor Zhang Yue.Methods The prescriptions of traditional Chinese medicine for lung cancer patients treated by Professor Zhang Yue,a famous Chinese medicine professor in Jilin Province,from March 2019 to December 2021,were collected and entered into the Ancient and Modern Medical Case Cloud Platform,and the frequency,taste,channel tropism,correlation and cluster relationship of the drugs used in these prescription were statistically analyzed.Results A total of 248 patients were included in the medical cases,with an equal proportion of male and female patients,and their age ranged from 50 to 80 years old,with the youngest being 26 and the oldest 90.There were 16 drugs with a frequency of≥80,among which thunberbg fritillary bulb,milkvetch root,largehead atractylodes rhizome,fi nger citron,etc.belong to the high-frequency items.Most of the drugs taste bitter,sweet and pungent,and are mild in terms of their natures,and mainly enter into the lung,spleen and stomach,and liver channels.The treatment methods are mainly clearing heat and removing toxicity,eliminating dampness and phlegm,promoting the production of fl uid and nourishing blood,invigorating qi for ascending yang,etc.Conclusion In his treatment of lung cancer with traditional Chinese medicine,Professor Zhang Yue upholds a holistic idea for syndrome differentiation,emphasizing giving consideration to both strengthening the body resistance and eliminating pathogens,and coordinating the cold and heat.As for strengthening the body resistance,it is stressed to tonify qi and invigorate the spleen,disperse stagnated liver qi and benefi t the lung.While in terms of eliminating pathogens,it is underscored to clear heat and remove toxicity,dissipate phlegm and resolve hard masses.
