摘要
目的:分析不同分型病毒性肺炎患者的免疫功能及细胞因子水平变化。方法:选取2020年1月—2022年12月山东省公共卫生临床中心收治的368例病毒性肺炎确诊患者,根据诊断标准分为轻型(n=115)、普通型(n=201)、危重型(n=52)三组。检测各组患者血清免疫功能及细胞因子指标,比较分析各组免疫功能及细胞因子的差异。结果:免疫功能检测指标中,CD3^(+)、CD4^(+)、CD8^(+)均呈下降趋势,轻型、普通型、危重型免疫功能检测指标比较,差异有统计学意义(P<0.05);细胞因子指标:IL-1β、IL-5、IL-6均成升高趋势,其中IL-1β、IL-6升高程度为危重型组>普通型组>轻型组,差异有统计学意义(P<0.05);三组IL-6/IL-10比值比较,差异无统计学意义(P>0.05)。结论:病毒性肺炎患者血清学T细胞(CD3^(+)、CD4^(+)、CD8^(+))相关指标明显下降。细胞因子中IL-1β,IL-6,IL-8表达水平有助于评估患者病情,IL-6/IL-10比值能够作为判断患者预后的预测指标,为临床治疗提供帮助。
Objective To study the correlation between the immune function and cytokine detection indexes of viral pneumonia and the severity and prognosis of the disease.Methods A total of 368 confirmed viral pneumonia patients admitted to Shandong Provincial Public Health Clinical Center from January 2020 to December 2022 were collected and divided into 3 groups according to the diagnostic criteria:the mild(115 cases),ordinary(201 cases)and critical(52 cases)groups.Serum immune function and cytokine indexes of patients in each group were detected,and the differences of immune function and cytokines in each group were compared and analyzed.Results The immune function indexes:CD3^(+),CD4^(+)and CD8^(+)all showed a decreasing trend,and there were statistically significant differences among the mild,ordinary and critical groups(P<0.05);Cytokine indexes:IL-1β,IL-5 and IL-6 were all increased,and the severity of IL-1βand IL-6 increased was the critical group>the ordinary group>the mild group(P<0.05);The IL-6/IL-10 ratio was significantly different in the mild,ordinary and critical groups.Conclusions Serological T-cell related indicators in viral pneumonia patients decreased significantly,mainly affecting T-cell function.The expression levels of IL-1β,IL-6 and IL-8 in cytokines are helpful to evaluate the patient’s condition,and the IL-6/IL-10 ratio can be used as a predictor of the prognosis of patients,providing help for clinical treatment.
