摘要
硬脊膜动静脉瘘(SDAVF)是一种罕见病,分析其临床特征,了解不常见瘘口的影像学特点及治疗方法,及早识别,减少漏诊、误诊。本研究回顾性分析了1例误诊为髓鞘少突胶质细胞糖蛋白(MOG)抗体病的硬脊膜动静脉瘘患者的临床表现、影像学特点、诊疗经过,并复习相关文献。本例患者男性,起初误诊为MOG抗体病,并给予激素冲击治疗,颈椎和胸椎磁共振提示颈段脊髓硬脊膜动静脉瘘,后行手术治疗,结合临床考虑硬脊膜动静脉瘘。延-颈交界区SDAVF可表现为脑干功能障碍,临床表现无特异性,对于原因不确定的脑干功能障碍,要考虑到SDAVF的可能性。
Spinal dural arteriovenous fistula(SDAVF)is a rare disease.Analyze its clinical characteristics,understand the imaging features and treatment methods of uncommon fistula,identify it as early as possible,and reduce missed diagnosis and misdiagnosis.The clinical manifestations,imaging features,diagnosis and treatment history of a patient with dural arteriovenous fistula misdiagnosed as myelin oligodendrocyte glyco-protein(MOG)antibody disease were retrospectively analyzed,and relevant literatures were reviewed.The male patient was misdiagnosed as MOG antibody disease at first,and was given steroid pulse therapy.Magnetic resonance imaging of the cervical and thoracic spine showed cervical spinal dural arteriovenous fistula.After surgery,the dural arteriovenous fistula was considered in combination with clinical considerations.SDAVF in the extension-cervical junction area can be manifested as brainstem dysfunction,and the clinical manifestations are non-specific.For brainstem dysfunction with uncertain causes,the possibility of SDAVF should be considered.
