摘要
目的 分析研究肺超声(Lung ultrasound, LUS)评分联合血浆miR-152-3p表达对急性肺损伤(acute lung injury, ALI)患者预后的价值。方法 以2019年4月-2021年5月海南医学院第二附属医院重症监护室收治的100例ALI患者为研究对象,根据患者生存情况将患者分为死亡组(28例)与生存组(72例),采用急性生理与慢性健康评估Ⅱ(Acute Physiology and Chronic Health EvaluationⅡ,APACHEⅡ)评分系统评估患者发病当天APACHEⅡ值,根据得分情况将患者分为高危组(39例)与低中危组(61例)。qRT-PCR检测miR-152-3p在患者血浆中的表达水平,并检测LUS评分。Logistic回归分析评价LUS评分与miR-152-3p水平是否作为ALI患者死亡的独立预测因素。受试者工作特征曲线(receiver operator characteristic curve,ROC)分析LUS评分、miR-152-3p水平及联合预测ALI患者死亡的价值。结果 与死亡组相比,生存组患者的LUS评分与miR-152-3p表达水平均降低,差异有统计学意义(P<0.05)。与高危组相比,低中危组患者的LUS评分miR-152-3p表达水平均降低,差异有统计学意义(P<0.05)。ROC曲线分析显示LUS评分联合miR-152-3p表达的AUC值大于二者单独诊断。LUS评分与血浆miR-152-3p表达水平可以作为ALI患者预后的独立危险因子。死亡组中LUS评分与血浆miR-152-3p表达水平呈正相关(r=0.571,P=0.002),生存组中LUS评分与血浆miR-152-3p表达水平无显著相关性(r=-0.023,P=0.851)。结论 LUS评分联合血浆miR-152-3p表达水平与ALI患者的病情与预后显著相关,二者联合对ALI患者预后评价具有较高预测价值。
Objective To investigate the prognostic value of lung ultrasound(LUS)score combined with plasma miR-152-3p expression in patients with acute lung injury(ALI).Methods A total of 100 ALI patients admitted to the hospital from April 2019 to May 2021 were selected as the research objects.According to the survival of the patients,the patients were divided into death group(28 cases)and survival group(72 cases).The Acute Physiology and Chronic Health EvaluationⅡ(APACHEⅡ)scoring system was used to evaluate APACHEⅡvalue of the patients on the day of onset.According to the scores,the patients were divided into a high-risk group(39 cases)and a low-intermediate risk group(61 cases).The qRT-PCR was used to detect the expression level of miR-152-3p in the plasma of the patients,and to detect LUS score.Logistic regression analysis evaluated whether LUS score and miR-152-3p level were independent predictors of death in ALI patients.The receiver operator characteristic curve(ROC)was used to analyze LUS score,the level of miR-152-3p and the combined value of predicting the death of ALI patients.Results Compared with death group,LUS score and the expression level of miR-152-3p in the survival group were significantly decreased(P<0.05).Compared with the high-risk group,the expression level of miR-152-3p in the low-moderate risk group was significantly lower(P<0.05).ROC curve analysis showed that the AUC value of LUS score combined with miR-152-3p expression was greater than that of the two alone.LUS score and plasma miR-152-3p expression levels could be used as independent risk factors for the prognosis of the ALI patients.There was a positive correlation between LUS score and plasma miR-152-3p expression level in death group(r=0.571,P=0.002),but there was no significant correlation between the LUS score and the plasma miR-152-3p expression level in survival group(r=-0.023,P=0.851).Conclusion LUS score combined with plasma miR-152-3p expression level is significantly correlated with the condition and prognosis of the ALI patie
