肱动脉内皮依赖性舒张功能、血脂代谢与原发性高血压合并骨质疏松症患者骨密度的关系研究

Relationship between brachial artery endothelium-dependent relaxation function,blood lipid metabolism and bone mineral density in patients with essential hypertension and osteoporosis

摘要 目的 研究肱动脉内皮依赖性舒张功能(Flow-mediated vasodilation, FMD)、血脂代谢与原发性高血压(Essential hypertension, EH)合并骨质疏松症(Osteoporosis, OP)患者骨密度(Bone mineral density, BMD)的关系。方法 收集2020年4月-2022年3月秦皇岛市海港医院内分泌科收治的138例EH患者的临床资料。依据骨密度仪检测的T值将患者分为3组:A组[T值≥-1.0标准差(Standard deviation, SD),n=59]、B组(T值-2.5SD~-1.0SD,n=52)、C组(T值≤-2.5SD,n=27),比较3组FMD、血脂代谢指标及BMD;采用Pearson相关性分析法分析FMD、血脂代谢指标与BMD的相关性。结果 与A组比较,B组和C组FMD、高密度脂蛋白胆固醇(High density lipoprotein cholesterol, HDL-C)及第1~4腰椎(1st~4th lumbar vertebrae, L1~L4)和右侧股骨颈的BMD值降低,且C组低于B组,差异均有统计学意义(P<0.05);与A组比较,C组甘油三酯(Triglyceride, TG)水平升高,差异有统计学意义(P<0.05),C组与B组比较无统计学差异(P>0.05);与A组比较,B组和C组总胆固醇(Total cholesterol, TC)水平升高,且C组高于B组,差异均有统计学意义(P<0.05)。Pearson相关性分析显示,FMD水平与L1~L4、右侧股骨颈的BMD值正相关(P<0.05);HDL-C水平与L1~L4 BMD值正相关(P<0.05),与右侧股骨颈BMD值无相关性(P>0.05);TG、TC水平与L1~L4、右侧股骨颈的BMD值负相关(P<0.05)。结论 原发性高血压合并OP患者存在血管内皮功能减退和血脂代谢紊乱现象,且FMD、TG、TC与骨密度关系密切,可作为监测EH患者OP的参考指标。 Objective To study the relationship between brachial artery flow-mediated vasodilation(FMD)of brachial artery,blood lipid metabolism and bone mineral density(BMD)in patients with essential hypertension(EH)combined with osteoporosis(OP).Methods Clinical data were collected from 138 patients with EH admitted to the hospital from April 2020 to March 2022.The patients were divided into 3 groups according to the T value detected by bone densitometry:A group[T value≥-1.0 standard deviation(SD),n=59],B group(T value-2.5SD~-1.0SD,n=52),C group(T value≤-2.5SD,n=27).The brachial artery endothelium-dependent diastolic function(FMD),lipid metabolism indexes and BMD were compared among the three groups.The correlation between FMD,blood lipid metabolism indexes and BMD was analyzed by Pearson correlation analysis method.The predictive value of FMD and lipid metabolism indexes for EH with OP were evaluated by receiver operating characteristic curve(ROC)and area under the curve(AUC).Results Compared with group A,the FMD,high density lipoprotein cholesterol(HDL-C)and BMD values of 1st~4th lumbar vertebrae(L1~L4)and right femoral neck were lower in groups B and C,and the differences were lower in group C than in group B,and the difference were statistically significant(P<0.05).Triglyceride(TG)levels were higher in Group C compared to Group A,and the difference were statistically significant(P>0.05),while there was no statistical difference between Group C and Group B(P>0.05).Total cholesterol(TC)levels were higher in Groups B and C compared to Group A,and the difference were statistically significant(P<0.05).Pearson correlation analysis showed that FMD levels were positively correlated with BMD values of L1~L4 and right femoral neck(P<0.05).The level of HDL-C was positively correlated with L1~L4 BMD values(P<0.05),while it had no correlation with the BMD values of the right femoral neck(P>0.05).The levels of TG and TC were negatively correlated with BMD values of L1~L4 and the right femoral neck(P<0.05).Conclusion There are va
