摘要
目的系统评价扶正消瘤中药联合化疗治疗三阴性乳腺癌(Triple negative breast cancer,TNBC)的有效性及安全性。方法计算机检索数据库包括Web of Science、PubMed、the Cochrane Library、EMbase、中国知网(CNKI)、维普中文科技期刊全文数据库(VIP)、万方数据资源系统、中国生物医学文献服务系统(SinoMed)等,检索以扶正消瘤中药联合化疗为主要干预措施治疗TNBC的随机对照临床试验研究(Randomized Controlled Trial,RCT)。两位独立评价者经过文献筛选、纳入RCT偏倚风险评估、资料提取后,Revman进行定量系统分析。结果共纳入14项RCT,总计1179例TNBC患者,系统评价结果提示试验组在改善临床疗效(临床有效率、临床控制率、疾病进展情况)、生活质量(卡氏评分及其改善、下降情况)、肿瘤标志物(CEA、CA153、CA125)、预后(无进展生存期、中位)及不良反应发生(白细胞减少、血小板减少、恶心呕吐)等指标方面均优于对照组(P<0.05)。而在稳定疾病、改善免疫功能(CD4^(+)/CD8^(+))、中心粒细胞减少、骨髓抑制、腹泻、脱发及复发转移方面两组差异无统计学意义(P>0.05)。结论本研究表明扶正消瘤中药联合化疗治疗TNBC疗效更好,不良反应更少、预后更好。
Objective To systematically evaluate the efficacy and safety of herbal medicine for supporting healthy qi and eliminating tumor combined chemotherapy in the treatment of triple-negative breast cancer(TNBC).Methods The randomized controlled trials(RCTs)of TNBC treated by herbal medicine for supporting healthy qi and eliminating tumor combined chemotherapy were retrieved from the databases of Web of SCI,PubMed,the Cochrane Library,EMbase,CNKI,VIP,Wanfang Data and SinoMed.Two reviewers screened the literature,extracted data and evaluated the risk of bias of the study independently.RevMan was used for the quantitative systematic analysis.Results A total of 14 RCTs were obtained,including 1179 patients with TNBC.The results of meta-analysis showed that the improvement of clinical efficacy(ORR,DCR,PD),quality of life(KPS score,the improvement or decline of KPS score),tumor markers(CEA,CA153,CA125),prognosis[progression-free survival(PFS),median PFS],and the incidence of adverse reactions(leucopenia,thrombocytopenia,nausea and vomiting)in the experimental group was superior to that in the control group(P<0.05).There were no significant differences between the two groups in stabilizing the disease,improving immune function(CD4^(+)/CD8^(+)),neutropenia,myelosuppression,diarrhea,alopecia,and recurrence and metastasis(P>0.05).Conclusion The results of meta-analysis indicated that herbal medicine for supporting healthy qi and eliminating tumor combined chemotherapy exerts stronger efficacy,less adverse reaction and better prognosis.
