摘要
复发性流产(recurrent spontaneous abortion,RSA)作为生育时期女性的一种常见临床疾病,它的病因相对复杂,包括遗传因素、解剖因素、血栓前状态等。近年来大多认为其与血栓前状态有着较密切的关联。血栓前状态复发性流产即女性妊娠期出现的病理性血液高凝状态,其属中医学“滑胎”等范畴,以“肾虚血瘀”为其主要的病因病机,治疗方面中医学多采用补肾活血、固护胎元为治疗原则,同时近年来临床运用隔姜灸、针灸疗法等外治法对此病有显著疗效。西医则多应用低剂量阿司匹林及低分子肝素等抗凝剂进行治疗,但临床不良反应时有发生,目前多采用中西医联合用药,效果较好。此外对于本病的诊断国内外目前仍无统一诊断标准。现将从中西医方面论治血栓前状态所致复发性流产的研究进展做一综述。
Recurrent spontaneous abortion(RSA)is a common clinical disease in women during childbearing.Its causes are relatively complex,including genetic factors,anatomical factors,prethrombotic state,etc.In recent years,most people think that it is closely related to the prethrombotic state.Recurrent abortion in the prethrombotic state,that is,the pathological blood hypercoagulation state in women during pregnancy.It belongs to the category of“sliding fetus”in traditional Chinese medicine,with“kidney deficiency and blood stasis”as the main etiology and pathogenesis.In terms of treatment,Chinese medicine mostly adopts kidney tonifying and activating blood circulation and protecting fetal elements as the treatment principles.At the same time,ginger moxibustion and acupuncture have been used clinically in recent years.External treatment methods such as therapy also have a significant effect on this disease.Western medicine mostly uses anticoagulants such as low-dose aspirin and low molecular weight heparin for treatment,but adverse clinical reactions occur from time to time.At present,most traditional Chinese and Western medicine are used together,and the effect is better.In addition,there is still no unified diagnostic standard for the diagnosis of this disease at home and abroad.The research progress in the treatment of recurrent abortion caused by prethrombotic state from the perspective of traditional Chinese and Western medicine is reviewed as follows.
