摘要
目的:评价富血小板血浆(platelet-rich plasma,PRP)疗法联合腰交感神经节射频热凝术治疗糖尿病足溃疡的临床疗效。方法:选择2021年1月7日至2022年3月31日于上海市浦东医院疼痛科接受治疗的40例糖尿病足溃疡病人,按随机数字表法分为两组,各20例,射频组(R组)采用腰交感神经节射频热凝术,联合组(P组)在射频组基础上联合PRP治疗。记录两组病人治疗前和治疗后1、2、4、6周的创面体积、疼痛数字分级评分法(numerical rating scale,NRS)评分、Bates Jensen创面评分(Bates-Jensen wound assessment tool,BWAT)、糖尿病特异性生活质量量表(diabetes specific quality of life,DSQL)评分,同时记录治疗后6周的创面愈合率、临床疗效以及治疗期间不良反应。结果:两组病人治疗后各时间点的创面体积、NRS评分、BWAT评分、DSQL评分较治疗前均改善(P<0.05)。P组在治疗后各时间点的创面体积、NRS评分、BWAT评分、DSQL评分明显低于R组(P<0.05)。随访至治疗后6周,P组在创面愈合率和临床疗效方面优于R组(P<0.05)。两组均未出现严重的不良反应。结论:富血小板血浆疗法联合腰交感神经节射频热凝术较单纯的腰交感神经节射频热凝术能够更好地促进糖尿病足溃疡愈合,缓解疼痛,提高病人的生活质量。
Objective:To evaluate the clinical efficacy of platelet-rich plasma(PRP)therapy combined with radiofrequency thermocoagulation of lumbar sympathetic ganglion in the treatment of diabetic foot ulcer.Methods:From January 7,2021 to March 31,2022,40 patients with diabetic foot ulcer in the Pain Department of Shanghai Pudong Hospital were divided into two groups according to a random number table,20 patients in each group.Radiofrequency group(group R)received radiofrequency thermocoagulation of lumbar sympathetic ganglion therapy,while combined treatment group(group P)received PRP therapy combined with radiofrequency thermocoagulation of lumbar sympathetic ganglion therapy.The wound volume,numerical rating scale(NRS),Bates-Jensen wound assessment tool(BWAT)and diabetes specific quality of life(DSQL)of patients before treatment and 1,2,4 and 6 weeks after treatment were recorded.Wound healing rate,clinical efficacy at 6 weeks after treatment and adverse reactions during treatment were also recorded.Results:The wound volume,NRS score,BWAT score and DSQL score of the two groups were lower at each time point after treatment than those before treatment(P<0.05).Compared with group R,the wound volume,NRS score,BWAT score and DSQL score of group P were significantly lower at all time points after treatment(P<0.05).Group P was superior to group R in wound healing rate and clinical efficacy at 6 weeks after treatment(P<0.05).No serious adverse reactions occurred in both groups.Conclusion:PRP therapy combined with radiofrequency thermocoagulation of lumbar sympathetic ganglion for diabetic foot ulcer demonstrated better improvement in the diabetic foot ulcer healing,pain and quality of life compared to radiofrequency thermocoagulation of lumbar sympathetic ganglion alone.
