摘要
目的:探讨二尖瓣病变合并轻度功能性三尖瓣关闭不全患者采用二尖瓣置换术同期三尖瓣成形术的应用效果。方法:选取2021年1月-2022年1月广东省高州市人民医院收治的二尖瓣病变合并轻度功能性三尖瓣关闭不全患者60例为观察对象,依据随机数字表法分为对照组与观察组,各30例。对照组采用二尖瓣置换术治疗,观察组在对照组基础上联合三尖瓣成形术治疗,比较两组患者心功能、心脏一般指标[右房内径(RA)、右室舒张末径(RV)、三尖瓣舒张期瓣环径(DTV)、肺动脉高压(SPAP)]、随访情况。结果:术后,观察组Ⅰ级、Ⅱ级心功能患者比例高于对照组,Ⅲ级、Ⅳ级心功能患者比例低于对照组,差异有统计学意义(P<0.001);术后,两组患者RA、RV、DTV、SPAP水平均降低,观察组低于对照组,差异有统计学意义(P<0.05);观察组存活率、三尖瓣轻度反流免除率高于对照组,三尖瓣反流复发率低于对照组,差异有统计学意义(P<0.05)。结论:在对二尖瓣病变合并轻度功能性三尖瓣关闭不全患者治疗期间,二尖瓣置换术同期三尖瓣成形术可显著改善心功能,延长患者存活时间。
Objective:To investigate the application effect of mitral valve replacement and tricuspid valvuloplasty in patients with mitral valve disease combined with mild functional tricuspid insufficiency.Methods:Sixty patients with mitral valve disease combined with mild functional tricuspid insufficiency admitted to Gaozhou People's Hospital from January 2021 to January 2022 were selected as the observation objects,and were divided into control group and observation group according to random number table method,with thirty cases in each group.The control group was treated with mitral valve replacement,and the observation group was treated with tricuspid valvuloplasty on the basis of control group.Cardiac function,general cardiac indexes[right atrial diameter(RA),right ventricular end-diastolic diameter(RV),tricuspid diastolic annular diameter(DTV),pulmonary arterial hypertension(SPAP)]and follow-up were compared between the two groups.Results:After surgery,the proportion of patients with gradeⅠand gradeⅡcardiac function in observation group was higher than that in control group,and the proportion of patients with gradeⅢand gradeⅣcardiac function was lower than that in control group,the difference was statistically significant(P<0.001).After surgery,RA,RV,DTV and SPAP levels in 2 groups were decreased,and the observation group was lower than the control group,the difference was statistically significant(P<0.05).The survival rate and exemption rate of mild tricuspid insufficiency in the observation group were higher than those in the control group,and the recurrence rate of tricuspid insufficiency was lower than that in the control group,with statistical significance(P<0.05).Conclusion:During the treatment of patients with mitral valve disease and mild functional tricuspid insufficiency,mitral valve replacement combined with tricuspid valvuloplasty can significantly improve cardiac function and prolong patient survival.
