摘要
目的 研究多层螺旋CT(MSCT)对腹部刺状异物的诊断价值。方法 回顾性分析2017.1~2020.1我院收治的经过手术或消化道内镜证实为腹部刺状异物的90例患者临床资料,均使用MSCT进行检查,通过容积重建(VR)、最大密度投影(MIP)、多平面重组(MPR)影像后处理技术进行三维成像,对比分析内镜及手术病理或临床治疗随访结果与影像诊断结果。结果 90例腹部刺状异物均表现有骨样高密度影,MSCT对腹部刺状异物诊断的敏感度及特异度均为100%。其中胃内异物24例,小肠异物42例,结肠异物17例,十二指肠2例,消化道外5例。经手术或内镜取出异物74例,其中53例异物为鱼刺,9例为牙签,9例为枣核异物,3例为不明骨性异物。结论 MSCT结合三维重组技术在腹部刺状异物的诊断中应用效果良好,可作为腹部刺状异物的首选检查方法,对于临床治疗有重要指导意义。
Objective To study the diagnostic value of multi-slice spiral CT(MSCT)on abdominal spinous foreign bodies.Methods A retrospective analysis was performed on clinical data of 90 patients who were confirmed to have abdominal spinous foreign bodies by surgery or gastrointestinal endoscopy in our hospital from January 2017 to January 2020,and they were given MSCT examination,and volume reconstruction(VR),maximum intensity projection(MIP)and multi-planar reconstruction(MPR)image post-processing technology were performed for three-dimensional imaging.The endoscopy and surgical pathology or clinical treatment follow-up results and imaging diagnosis results were compared and analyzed.Results All 90 cases of abdominal spinous foreign bodies showed bone-like high density shadow.The sensitivity and specificity of MSCT for the diagnosis of abdominal spinous foreign bodies were both 100%.Among them,there were 24 cases of foreign bodies in the stomach,42 cases of foreign bodies in the small intestine,17 cases of foreign bodies in the colon,2 cases of foreign bodies in the duodenum,and 5 cases of foreign bodies outside the digestive tract.74 cases of foreign bodies were removed by surgery or endoscopy,including 53 cases of fish bones foreign bodies,9 cases of toothpicks,9 cases of jujube pit foreign bodies,and 3 cases of unknown bony foreign bodies.Conclusion MSCT combined with three-dimensional reconstruction technology has a good application effect in the diagnosis of abdominal spinous foreign bodies.It can be used as the first choice for the examination of abdominal spinous foreign bodies,and has important guiding significance for clinical treatment.
