摘要
腰方肌阻滞(QLB)是一种在超声引导下将局麻药注射在腰方肌或其周围筋膜间隙的筋膜间平面阻滞技术,因其操作简单安全、镇痛效果确切、并发症少而备受麻醉医生的关注。外侧弓状韧带上QLB是最新提出的腰方肌穿刺入路,具有效果更可靠、起效更快、阻滞范围更广的特点。本文就腰方肌解剖及其与周围筋膜的毗邻关系、QLB的作用机制、不同QLB入路及优缺点、临床应用等进行综述。
The quadratus lumborum block(QLB)is an interfascial plane block technique in which local anaesthetic is injected into the quadratus lumborum or its surrounding fascial space under ultrasound guidance.It has attracted much attention from anesthesiologists because of its simple and safe operation,precise analgesic effect and few complications.The supra-arcuate ligament block is the latest proposed quadratus lumborum block puncture approach,which has more reliable results,faster onset of action and wider block coverage.This article reviews the anatomy of the quadratus lumborum and its adjacent relationship with the surrounding fascia,the mechanismof action of the quadratuslumborum block,the different quadratus lumborum block approaches and their advantages and disadvantages,and clinical applications.
