摘要
目的 观察益气化痰方+穴位敷贴对中晚期肺癌肺气虚证咳喘的疗效及血清肿瘤标志物细胞角蛋白19片段(CYFRA21-1)、糖类抗原199(CA199)、鳞癌相关抗原(SCC)及外周血辅助性T细胞17(Th17)/调节性T细胞(Treg)平衡的影响。方法 选取120例中晚期肺癌患者为对象,采用随机数字法分为对照组60例和观察组60例,对照组给予常规抗炎止咳化痰解痉平喘治疗,观察组给予益气化痰方联合穴位敷贴治疗,比较2组临床疗效,中医证候积分,肺功能[用力肺活量(FVC),第1秒用力呼气容积(FEV1),最大呼气流量(PEF)],血清CYFRA21-1、CA199、SCC,外周血Th17/Treg及炎性因子[白细胞介素(IL)-6、IL-10、嗜酸性粒细胞(EOS)]水平。结果 治疗后观察组缓解率高于对照组(93%与75%,P<0.05);治疗后观察组中医证候积分,血清CYFRA21-1、CA199、SCC,外周血Th17/Treg及IL-6、EOS水平均低于对照组,FEV1、FVC、PEF和IL-10水平高于对照组(P<0.05)。结论 益气化痰方联合穴位敷贴治疗中晚期肺癌肺气虚证咳喘疗效显著,可降低血清CYFRA21-1、CA199、SCC水平,改善肺功能及Th17/Treg细胞失衡状态。
Objective To observe the therapeutic effect of Yiqi Huatan recipe combined with acupoint application on cough and asthma of lung qi deficiency syndrome in advanced lung cancer,and the influence on serum tumor markers cytokeratin 19 fragment(CYFRA21-1),carbohydrate antigen 199(CA199),squamous cell carcinoma related antigen(SCC)and peripheral blood T helper 17(Th17)/regulatory T cell(Treg)balance.Methods A total of 120 patients with advanced lung cancer were enrolled in this study and randomly divided into control group(n=60)and observation group(n=60).The control group received conventional anti-inflammatory treatments for relieving cough,reducing phlegm,getting spasmolysis and relieving asthma,while the observation group received Yiqi Huatan recipe combined with acupoint application.The two groups were compared in terms of clinical effects,traditional Chinese medicine(TCM)syndrome scores,lung function[forced vital capacity(FVC),forced expiratory volume in 1 second(FEV1),peak expiratory flow(PEF)],serum CYFRA21-1,CA199,SCC,peripheral blood Th17/Treg and inflammatory factors[interleukin-6(IL-6),IL-10,eosinophils(EOS)]levels.Results After treatment,the remission rate in the observation group was higher than that in the control group(93%vs 75%)(P<0.05).After treatment,the observation group had lower TCM syndrome scores,serum CYFRA21-1,CA199,SCC,Th17/Treg,IL-6 and EOS levels,and higher FEV1,FVC,PEF and IL-10 than those in the control group(P<0.05).Conclusion Yiqi Huatan recipe combined with acupoint application is effective in treating cough and asthma of lung qi deficiency syndrome in advanced lung cancer.The combined treatment can reduce serum CYFRA21-1,CA199,and SCC levels,and improve lung function and Th17/Treg imbalance.
