摘要
目的:分析探讨输尿管硬镜(rigid ureteroscopy,R-URS)与输尿管软镜(flexible ureteroscopy,F-URS)治疗输尿管上段结石的效果和安全性。方法:选取2019年6月-2022年1月抚州市第一人民医院收治的60例输尿管上段结石患者。根据患者治疗方式的差异将患者分为两组,甲组患者均采取输尿管硬镜治疗,而乙组患者均采取输尿管软镜治疗。比较两组手术时间、住院费用、住院时间、并发症发生率、结石清除率、一次碎石成功率。结果:两组手术时间和住院时间比较,差异均无统计学意义(P>0.05)。甲组一次碎石成功率和术后2周结石清除率均低于乙组(P<0.05)。两组术后4周结石清除率和术后半年结石清除率比较,差异均无统计学意义(P>0.05)。甲组的住院费用为(14 423.28±2 834.23)元,少于乙组的(25 734.34±4 234.22)元(P<0.05)。两组患者均未出现有感染性休克和死亡的病例,两组术后发热、止痛药使用、石街形成、黏膜挫伤发生率比较,差异均无统计学意义(P>0.05)。结论:R-URS与F-URS在治疗输尿管上段结石方面均具有较高的安全性,与R-URS相比,F-URS的一次碎石成功率和术后2周结石清除率更高,可以作为临床治疗的首选方案,值得在临床上加以推广运用。
Objective:To analyze the efficacy and safety of rigid ureteroscopy(R-URS)and flexible ureteroscopy(F-URS)in the treatment of upper ureteral calculi.Method:A total of 60 patients with upper ureteral calculi admitted to the First People's Hospital of Fuzhou City from June 2019 to January 2022 were selected,they were divided into two groups according to the differences in treatment methods.Patients in group A were treated with rigid ureteroscopy,while patients in group B were treated with flexible ureteroscopy.The surgical time,hospital cost,hospital stay,complication rate,stone clearance rate and success rate of lithotripsy at one time were compared between the two groups.Result:There were no significant differences in surgical time and hospital stay between the two groups(P>0.05).The success rate of lithotripsy at one time and 2 week after surgery stone clearance in group A were lower than those in group B(P<0.05).There were no significant differences in stone clearance rates between the two groups at 4 weeks and 6 months after surgery(P>0.05).The hospitalization cost in group A was(14423.28±2834.23)yuan,which was less than(25734.34±4234.22)yuan in group B(P<0.05).There were no cases of septic shock or death in both groups,and there were no significant differences in the incidences of postoperative fever,painkiller use,stone street formation and mucosal contusion between the two groups(P>0.05).Conclusion:Both R-URS and F-URS have high safety in the treatment of upper ureteral calculi.Compared with R-URS,F-URS has higher success rate of lithotripsy at one time and stone clearance rate of 2 weeks,and can be used as the first choice for clinical treatment,which is worthy of clinical application.
