摘要
目的:探讨Ⅰ型和Ⅱ型剖宫产瘢痕妊娠(CSP)通过高强度聚焦超声(HIFU)与子宫动脉栓塞术(UAE)治疗的效果。方法:回顾性分析2020年1月-2021年12月亳州市谯城兴华医院收治的140例CSP患者,其中75例行HIFU治疗后在B超引导下行刮宫术,作为试验组,65例行UAE后在B超引导下行刮宫术,作为对照组。比较两组临床指标[术后即刻视觉模拟评分法(VAS)评分、术中出血量、住院时间、不良反应]及术后恢复情况[β-人绒毛膜促性腺激素(β-hCG)及月经恢复正常时间、月经量减少发生率、阴道出血持续时间]。结果:两组术中出血量比较,差异无统计学意义(P>0.05);与对照组相比,试验组的VAS评分较低,住院时间较短,不良反应发生率较低,差异均有统计学意义(P<0.05);两组β-hCG及月经恢复正常时间比较,差异均无统计学意义(P>0.05);试验组月经量减少发生率低于对照组,阴道出血持续时间短于对照组,差异均有统计学意义(P<0.05)。结论:与UAE相比,行HIFU治疗后在B超引导下刮宫术治疗CSP具有术后疼痛轻、住院时间短、术后月经量减少及不良反应发生率低的优势。
Objective:To investigate the therapeutic effects of high intensity focused ultrasound(HIFU)and uterine artery embolization(UAE)for typeⅠandⅡcesarean scar pregnancy(CSP).Method:A retrospective analysis was performed on 140 patients with CSP who were admitted to Qiaocheng Xinghua Hospital of Bozhou from January 2020 to December 2021.Among them,75 cases received dilatation and curettage under B-ultrasound guidance after HIFU treatment,as the experimental group,65 cases received dilatation and curettage under B-ultrasound guidance after UAE,as the control group.Clinical indexes[visual analogue scale(VAS)scores immediately after operation,intraoperative bleeding volume,hospitalization time,adverse reactions]and postoperative recovery[the time ofβ-human chorionic gonadotropin(β-hCG)and menstruation to return to normal,the incidence of decreased menstrual flow and duration of vaginal bleeding]were compared between the two groups.Result:There was no significant difference in intraoperative bleeding volume between the two groups(P>0.05).Compared with the control group,the VAS score of the experimental group was lower,the hospitalization time was shorter,and the incidence of adverse reactions was lower,the differences were statistically significant(P<0.05).There were no significant differences in the time ofβ-human chorionic gonadotropin(β-hCG)and menstruation to return to normal between the two groups(P>0.05).The incidence of decreased menstrual flow in the experimental group was lower than that in the control group,and the duration of vaginal bleeding was shorter than that in the control group,the differences were statistically significant(P<0.05).Conclusion:Compared with UAE,curettage for CSP under the guidance of B-ultrasound after HIFU treatment has the advantages of mild postoperative pain,short hospitalization time,low incidence of decreased menstrual flow after operation and adverse reactions.
