摘要
目的研究前路钢板内固定治疗颈椎骨折脱位的效果及并发症影响因素。方法回顾性分析2016年3月-2021年10月200例颈椎骨折脱位患者资料研究,所有患者行前路钢板内固定,观察和记录手术时间、术中失血量、植骨融合时间,比较术前术后JOA评分、术后Cobb角、术后D值情况。比较术后并发症情况,分析术后并发症的影响因素。结果术后患者JOA评分、Cobb角、D值显著高于术前,神经损伤恢复程度好于术前(P<0.05)。单因素分析显示,有无并发症患者在术中失血量、植骨融合时间、手术时间、术后JOA评分、术后D值、性别、术后Cobb角、合并症差异显著(P<0.05)。多因素分析结果显示,手术时间、术中失血量、合并症、麻醉情况、术后Cobb角是影响并发症的危险因素。结论前路钢板内固定治疗颈椎骨折脱位效果显著,可提升颈椎功能,促进关节复位,且手术时间、术中出血量、术后Cobb角、合并症、麻醉情况是影响术后并发症的危险影响因素。
Objective To study the effect of anterior plate internal fixation in the treatment of cervical fracture and dislocation and the influencing factors of complications.Methods The data of 200 patients with cervical fracture and dislocation from March 2016 to October 2021 were analyzed retrospectively.All patients underwent anterior plate internal fixation.The operation time,intraoperative blood loss and bone graft fusion time were observed and recorded.The preoperative and postoperative JOA score,postoperative Cobb angle and postoperative D value were compared.The postoperative complications were compared and the influencing factors of postoperative complications were analyzed.Results After operation JOA score,Cobb angle and D value of the patients were significantly higher than those before operation,and the recovery degree of nerve injury was better than that before operation(P<0.05).Univariate analysis showed that there were significant differences in intraoperative blood loss,bone graft fusion time,operation time,postoperative JOA score,postoperative D value,gender,postoperative Cobb angle and complications in the patients with and without complications(P<0.05).Multivariate analysis showed that operation time,intraoperative blood loss,complications,anesthesia and postoperative Cobb angle were the risk factors of complications.Conclusion Anterior plate internal fixation is remarkably effective in the treatment of cervical fracture and dislocation,improves cervical function and promotes joint reduction.The operation time,intraoperative bleeding,postoperative Cobb angle,complications and anesthesia are the risk factors of postoperative complications.
