摘要
目的探究盆底磁刺激联合电刺激在治疗根治性前列腺切除术后尿失禁的临床疗效。方法选取2021年8月至2022年6月在本院接受根治性前列腺切除术且术后存在尿失禁的患者90例,经Ingelman-Sundberg分度法确定为轻中度尿失禁,按照随机数字表法将其分为三组,A组实施盆底磁刺激(每周2次)治疗,B组实施盆底电刺激(每周2次)治疗,C组实施盆底磁电联合刺激(每周2次)治疗。比较三组患者治疗后尿失禁的恢复情况,客观测量指标包括尿动力学相关指标、1 h尿垫重量测试、ICIQ-SF评分及生活质量评分。结果C组在治疗后3个月的临床疗效明显高于A、B两组,差异具有统计学意义(P<0.05),C组在治疗后的漏尿量明显少于A、B两组,差异具有统计学意义(P<0.05)。C组的ICI-Q-SF及I-QOL评分明显优于A、B两组,差异具有统计学意义(P<0.05)。结论盆底磁刺激联合电刺激能够明显提高根治性前列腺切除术后轻中度尿失禁的疗效,其在尿控恢复方面尤为显著。不良反应少,复发率低。
Objective To investigate the efficacy of pelvic magnetic stimulation combined with electrical stimulation in the treatment of urinary incontinence after radical prostatectomy.Methods 90 patients in our hospital who was undergoing radical prostatectomy and urinary incontinence were choosed.The patients were classified as mild to moderate urinary incontinence by Ingelman-Sundberg grading method,according to the random number table method.They were divided into three groups,the group A received pelvic floor magnetic stimulation(twice a week),the group B received pelvic floor electric stimulation(twice a week)and the group C received combined pelvic floor magnetic stimulation(twice a week).Comparison of three groups of patients with urinary incontinence was performed after the treatment.The objective measures included urodynamic indicators,1h pad weight test,ICI-QSF score and quality of life score.Results The clinical efficacy of the group C was significantly higher than that of the group A and the group B 3 months after the treatment,the difference was statistically significant(P<0.05).The amount of urine leakage in the group C after the treatment was significantly less than that in the group A and the group B,the difference was statistically significant(P<0.05).ICI-Q-SF and I-QOL score of the group C were significantly better than the group A and group B,and the difference was statistically significant(P<0.05).Conclusion Pelvic floor magnetic stimulation combined with electrical stimulation can significantly improve the efficacy of mild to moderate urinary incontinence after radical prostatectomy and it is particularly significant in the recovery of urinary control.It has less adverse reactions,low recurrence rate,and is worthy of clinical promotion.
