摘要
目的:探讨振幅整合脑电图(aEEG)对新生儿低血糖脑损伤的早期诊断和预后评估的价值。方法:选择2021年5月至2022年8月在我院NICU住院的60例低血糖新生儿作为观察组,另选取同期本院出生60例无脑损伤的新生儿作为对照组。比较观察组与对照组aEEG监测结果,并分析aEEG监测结果异常程度与患儿患病风险的相关性。根据神经评分法(NBNA)评分结果将观察组患儿细分为预后良好组与预后较差组,比较两组aEEG监测结果,并分析aEEG监测结果异常程度与患儿预后的相关性。结果:观察组与对照组aEEG背景活动与连续性评估结果存在明显差异(P<0.05);预后良好组与预后较差组aEEG背景活动与连续性评估结果存在明显差异(P<0.05)。相关性分析结果显示,aEEG背景活动异常程度及连续性异常程度均与新生儿低血糖脑损伤的发病风险呈正相关(P<0.05);且与新生儿低血糖脑损伤的预后结局负相关(P<0.05)。结论:新生儿低血糖脑损伤aEEG监测结果与未发生脑损伤的新生儿存在较大差异,临床可通过对新生儿进行aEEG监测来评估其低血糖脑损伤的发生风险,同时还能对患儿治疗结局进行预测,为相关治疗方案的制定与改善提供充分数据参考。
Objective:To investigate the value of amplitude integrated electroencephalography(aEEG)in the early diagnosis and prognosis of hypoglycemic brain injury in neonates.Methods:Sixty neonates with hypoglycemia who were hospitalized in NICU of our hospital from May 2021 to August 2022 were selected as the observation group,and sixty neonates without brain injury during the same period were selected as the control group.The results of aEEG monitoring were compared between the observation group and the control group,and the correlation between the abnormal degree of aEEG monitoring results and the risk of disease was analyzed.The observation group was subdivided into a good prognosis group and a poor prognosis group according to the neurological score(NBNA).The aEEG monitoring results of the two groups were compared,and the correlation between the abnormal aEEG monitoring results and the prognosis of the children was analyzed.Results:There were significant differences in aEEG background activity and continuity assessment between the observation group and the control group(P<0.05).There were significant differences in aEEG background activity and continuity assessment between good and poor prognosis groups(P<0.05).Correlation analysis showed that the degree of abnormal aEEG background activity and the degree of abnormal continuity were positively correlated with the risk of neonatal hypoglycemic brain injury(P<0.05).And it was negatively correlated with the outcome of neonatal hypoglycemic brain injury(P<0.05).Conclusions:The results of aEEG monitoring of hypoglycemic brain injury in neonates are significantly different from those of neonates without brain injury.The risk of hypoglycemic brain injury can be evaluated by aEEG monitoring of neonates in clinical practice,and the treatment outcome of neonates can be predicted,providing sufficient data reference for the formulation and improvement of relevant treatment programs.
