摘要
骨关节炎(OA)是老年人常见的退行性疾病,以膝关节尤为常见,又称膝骨关节炎(KOA),目前临床上主要以早期对症治疗、晚期关节置换的方案进行治疗,尚无有效手段延缓KOA进展。近年来,膝关节滑膜病变中的渗液性滑膜炎的诊治引起了很多研究者的关注。临床上常用超声检查及磁共振成像(MRI)检查对其进行诊断,又以MRI检查结果较为准确。研究发现膝关节渗液性滑膜炎的发生、发展会加速KOA进展,加重KOA的临床表现。有相关临床研究发现对患有膝关节渗液性滑膜炎的患者进行治疗,无论患者是否伴有KOA,均可在一定程度上延缓KOA进展,改善KOA的临床表现,但对于该疾病的治疗方案仍未达成共识。本文对膝关节渗液性滑膜炎临床表现、治疗及其与KOA关系的研究进展进行综述。
Osteoarthritis(OA)is a common degenerative disease in the elderly,especially in the knee joint,also known as knee osteoarthritis(KOA).At present,main clinical treatments are early symptomatic treatment and advanced joint replacement.There is no effective way to slow down KOA progression.In recent years,the diagnosis and treatment of knee effusion synovitis have attracted the attention.Ultrasound and magnetic resonance imaging(MRI)are commonly used for clinical diagnosis of knee effusion synovitis,and the results of MRI are more accurate.It has been found that the development of knee effusion synovitis accelerates KOA progression and exacerbates clinical manifestations of KOA.Clinical studies have found that treatment of knee effusion synovitis,whether accompanied by KOA or not,can slow the progression of KOA and improve its clinical manifestations to some extent,but there is still no accepted treatment option for this disease.In this paper,we reviewed clinical manifestations and treatment of knee effusion synovitis and its relationship with KOA.
