摘要
膝单髁置换术(UKA)的适应证自其问世以来一直面临着扩展的挑战,有些观点正在颠覆人们的认知。无论在传统或现代的UKA患者选择中,前交叉韧带缺失(ACLD)与严重关节畸形一直都被认为是禁忌,尤其是ACLD被认为是绝对禁忌,也是术中UKA转为全膝关节置换术(TKA)的决策依据。近年来,不断有关于ACLD及严重关节畸形的膝骨关节炎UKA获得成功的报道。对此,不能盲从,要认真分析与研判,避免将个体经验当做普遍真理加以推广,避免走入UKA患者选择的误区。在UKA患者的选择中应坚守原则,选择更合适的患者,追求最佳的手术结果。
The indication of unicompartmental knee arthroplasty(UKA)has been facing an expanding challenge since its advent,and some viewpoints are overturning people's cognition.Anterior cruciate ligament deficiency(ACLD)and severe joint deformity have always been considered as contraindications,especially ACLD is considered as an absolute contraindication,which is also the decisionmaking basis for intraoperative UKA to total knee arthroplasty(TKA)conversion.In recent years,there have been reports of successful UKA experience in ACLD and severe joint deformity.In this regard,we should not follow blindly,but carefully analyze and judge,and avoid promoting individual experience as a universal truth and entering into the misunderstanding in patient selection for UKA.We should stick to the principle in the selection of patients for UKA.The more suitable patients you select,the best surgical results you will get.
