摘要
目的 探讨独步汤联合常规西药治疗类风湿关节炎(RA)肝肾不足证的有效性和安全性。方法 采用随机数字表法将60例患者分为试验组和对照组各30例。对照组予甲氨蝶呤片,10 mg/次,1次/周,口服;艾拉莫德片,25 mg/次,2次/d,口服;叶酸片,10 mg/次,1次/周,口服;按需口服美洛昔康片。试验组在对照组基础上予独步汤,每日1剂,早晚分服。2组均连续治疗12周。观察2组临床疗效,比较2组中医证候积分、关节肿胀数、关节压痛数、晨僵时间、视觉模拟评分法评分(VAS)、28个关节的疾病活动度评分(DAS28)、红细胞沉降率(ESR)、超敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6),监测止痛药(美洛昔康片)应用情况及不良反应。结果 试验组总有效率为96.67%(29/30),对照组为76.67%(23/30),2组比较差异有统计学意义(P<0.05)。与治疗前比较,2组治疗后中医证候积分、关节肿胀数、关节压痛数、晨僵时间、VAS评分、DAS28及ESR、hs-CRP、IL-6水平均明显降低(P<0.05);2组治疗后比较,试验组上述指标均明显低于对照组(P<0.05)。2组美洛昔康片用药人数比率相当,试验组用药频次比率明显低于对照组(P<0.05)。试验组不良反应率低于对照组(P<0.05)。结论 独步汤联合常规西药治疗RA肝肾不足证可提高患者临床疗效,改善临床症状及炎症反应,降低不良反应率。
Objective To explore the efficacy and safety of Dubu Decoction combined with conventional Western medicine in the treatment of rheumatoid arthritis(RA) with liver and kidney deficiency syndrome.Methods Totally 60 patients were divided into experimental group and control group according to random number table method,with 30 cases in each group.The control group was given methotrexate tablets,orally,10 mg/time,once a week;iguratimod tablets,25 mg/time,twice/day,orally;folic acid tablets,10 mg/time,once a week,orally;oral meloxicam tablets as needed.On the basis of the control group,the experimental group was treated with Dubu Decoction,one dosage a day,taken in the morning and evening.The course of treatment of both groups was 12weeks.The clinical efficacy of both groups was observed.TCM syndrome score,joint swelling number,joint tenderness number,morning stiffness time,VAS,DAS28,ESR,hs-CRP,and IL-6 of the two group were compared.Pain medication(meloxicam) application and adverse reactions were monitored.Results The total clinical effective rate was 96.67%(29/30) in the experimental group and 76.67%(23/30) in the control group.There was significant difference between the two groups(P<0.05).Compared with before treatment,the TCM syndrome score,joint swelling number,joint tenderness number,morning stiffness time,VAS,DAS28,ESR,hs-CRP and IL-6 of the two groups were significantly lower after treatment(P<0.05).After treatment,the above indicators in the experimental group were significantly lower than those in the control group(P<0.05).The retio of meloxicam tablets users in the two groups was similar,and the frequency of medication in the experimental group was significantly lower than that in the control group(P<0.05).The adverse reaction rate in the experimental group was lower than that in the control group(P<0.05).Conclusion Dubu Decoction combined with conventional Western medicine in the treatment of RA with liver and kidney deficiency syndrome can promote the clinical efficacy,improve clinical symptoms and infla
