摘要
目的研究2型糖尿病(type 2 diabetes mellitus,T2DM)患者单核细胞-淋巴细胞比值(monocyte-lymphocyte ratio,MLR)与糖尿病足(diabetic foot,DF)之间的相关性。方法回顾性分析2016年1月至2022年5月右江民族医学院附属医院收治的296例T2DM患者,其中不合并DF的患者135例(T2DM组),合并DF的患者中,Wagner分级0~2级的患者95例(轻度DF组),Wagner分级3~5级的患者66例(重度DF组)。通过比较各组的MLR,探索T2DM患者MLR与DF之间的相关性。结果轻度DF组、重度DF组MLR均高于T2DM组,重度DF组MLR高于轻度DF组(P<0.001)。MLR与Wagner DF等级呈正相关(P<0.001);多因素logistic回归分析显示,MLR可能是DF的独立危险因素(调整后OR=8.738,95%CI:1.393~54.808,P<0.05)。ROC曲线分析表明,MLR预测DF患者的曲线下面积为0.747(P<0.001)。结论DF患者MLR显著升高,并与Wagner等级呈正相关,MLR可能是DF的独立危险因素,并具有一定的预测价值。
Objective To investigate correlation between monocyte-lymphocyte ratio(MLR)and diabetic foot(DF)in patients with type 2 diabetes mellitus(T2DM).Methods A retrospective analysis was carried out on 296 patients with T2DM admitted to the Affiliated Hospital of Youjiang Medical University for Nationalities from January 2016 to May 2022,including 135 patients without DF(T2DM group),95 patients with DF with Wagner grading of 0 to 2(mild DF group),and 66 patients with DF with Wagner grading of 3 to 5(severe DF group).The correlation between MLR and DF in T2DM patients was explored by comparing MLR in each group.Results The MLRs of the mild DF and severe DF groups were higher than that of the T2DM group,whereas the MLR of the severe DF group was higher than that of the mild DF group(P<0.001).MLR was positively correlated with the Wagner DF grade(P<0.001).Multivariate logistic regression analysis showed that MLR might be an independent risk factor for DF(adjusted OR=8.738,95%CI:1.393-54.808,P<0.05).Receiver operating characteristic(ROC)curve analysis showed that the area under the curve of the MLR for predicting diabetic foot ulcer was 0.747(P<0.001).Conclusion MLR significantly increases in DF patient and is positively correlated with Wagner grade.MLR may be an independent risk factor for DF,and it has certain predictive value.
