摘要
目的:探讨超声引导技术在白血病患儿治疗中应用,通过与盲穿法成功率及并发症进行对比,可发现超声引导技术可有效提高经肘正中静脉置入外周静脉导管(PICC)的成功率及减少并发症,从而体现出超声引导穿刺的优越性。方法:选择2011年3月-2018年9月收治的白血病患儿264例为研究对象,其中在超声引导下经肘正中静脉置入PICC的135例新生儿为观察组,盲穿法置入PICC的129例为对照组。比较两组的一次置管成功率、静脉炎、导管堵塞及穿刺点出血发生率。结果:观察组一次穿刺成功率显著高于对照组(P<0.01);观察组静脉炎发生率明显低于对照组(P<0.01);观察组导管堵塞发生率显著低于对照组(P<0.01);观察组穿刺点出血发生率显著低于对照组(P<0.01)。结论:白血病患儿在超声引导下经肘正中静脉置入PICC,与其他方法置入PICC相比,能显著提高一次性穿刺成功率、降低早期静脉炎、导管堵塞发生率及导管渗血发生率,值得各个医院在白血病患儿PICC置管中推广使用,可一定程度上减少病人费用及反复穿刺给患儿带来的痛苦。
Objective:To explore the application of ultrasonic guidance technology in the treatment of children with leukemia.By comparing with the success rate and complications of blind puncture,it can be found that ultrasonic guidance technology can effectively improve the success rate of peripheral venous catheter(PICC)catheterization through the median elbow vein and reduce the complications,thus reflecting the advantages of ultrasonic guided puncture.Methods:264 children with leukemia admitted to the hospital from March 2011 to September 2018 were selected as the research objects,and divided into two groups.135 cases in the observation group received PICC catheterization through median cubital vein,and 129 cases in the control group received PICC catheterization through blind penetration.The success rate of one puncture,phlebitis,catheter obstruction and bleeding at puncture point in the two groups were compared.Results:The success rate of one-time puncture in the observation group was significantly higher than that in the control group(P<0.01);The incidence of phlebitis in the observation group was significantly lower than that in the control group(P<0.01);The incidence of catheter obstruction in the observation group was significantly lower than that in the control group(P<0.1);The incidence of bleeding at puncture point in the observation group was significantly lower than that in the control group(P<0.01).Conclusion:Compared with other methods of PICC catheterization,ultrasound guided PICC catheterization through the median elbow vein in children with leukemia can significantly improve the success rate of one-time puncture,reduce the incidence of early phlebitis,catheter obstruction and catheter blood leakage.It is worth promoting the use of PICC catheterization in children with leukemia in hospitals,which can reduce the cost of patients and the pain caused by repeated puncture to a certain extent.
