摘要
目的观察围术期综合护理干预在预防妇科盆腔手术患者下肢深静脉血栓形成(LDVT)中的应用效果。方法选取2018年3月至2020年3月我院收治的100例行妇科盆腔手术治疗患者为研究对象,根据护理方法不同将其分为对照组(n=50,常规护理)和试验组(n=50,围术期综合护理)。比较两组的护理效果。结果试验组的术中出血量少于对照组,术后排气时间、进食时间、下床活动时间及切口愈合时间短于对照组,术后LDVT发生率低于对照组(P<0.05)。术后1周,试验组的细胞间粘附分子-1(ICAM-1)、白细胞介素-1β(IL-1β)、D-二聚体、纤维蛋白原(FIB)水平低于对照组,白细胞介素-10(IL-10)水平高于对照组(P<0.05)。试验组术后1周的心理状态、生活能力、社会功能、生理功能评分高于对照组(P<0.05)。结论围术期综合护理在预防妇科盆腔手术LDVT中的效果显著,可有效缩短患者的康复进程,改善预后,值得推广。
Objective To observe the application effect of perioperative comprehensive nursing intervention on prevention of lower extremity deep venous thrombosis(LDVT)in patients undergoing gynecological pelvic surgery.Methods A total of 100 patients who underwent gynecological pelvic surgery in our hospital from March 2018 to March 2020 were selected as the research objects.According to different nursing methods,the patients were divided into control group(n=50,routine nursing)and experimental group(n=50,perioperative comprehensive nursing).The nursing effects of the two groups were compared.Results The intraoperative blood loss in the experimental group was less than that in the control group,the postoperative exhaust time,feeding time,ambulation time and incision healing time were shorter than those in the control group,and the incidence of postoperative LDVT was lower than that in the control group(P<0.05).One week after operation,the levels of intercellular adhesion molecule-1(ICAM-1),interleukin-1β(IL-1β),D-dimer and fibrinogen(FIB)in the experimental group were lower than those in the control group,and the level of interleukin-10(IL-10)was higher than that in the control group(P<0.05).The scores of psychological state,living ability,social function and physiological function in the experimental group were higher than those in the control group at 1 week after operation(P<0.05).Conclusion Perioperative comprehensive nursing has a significant effect in preventing LDVT after gynecological pelvic surgery,it can effectively shorten the rehabilitation process of patients,and improve the prognosis,which is worth promoting.
