摘要
目的:探讨双相抑郁、单相抑郁和精神分裂症患者事件相关脑电位N400的差异及其临床实用意义。方法:采用汉语成语正常结尾字(匹配)与汉语成语结尾歧义字(非匹配)模式,对24例双相抑郁患者(双相抑郁组)、25例单相抑郁患者(单相抑郁组)、26例精神分裂症患者(精神分裂组)和28名健康对照者(对照组)作N400检测。结果:与对照组相比,3个患者组服药前N400潜伏期在额区(Fz)明显延迟,同时波幅下降(P<0.05或P<0.01)。在Fz脑区中,N400潜伏期延迟与一般状况分呈正相关(r=0.322,P=0.041)。N400波幅下降与阳性症状分(r=-0.398,P=0.007)和PANSS总分(r=-0.339,P=0.030)呈负相关。结论:N400可作为鉴别单相抑郁、双相抑郁和精神分裂症的实验室辅助参考手段之一。此结果有待于转化中进一步证实。
Objective:To explore the difference and its clinical significance of event-related potential N400 in patients with bipolar depression,unipolar depression and schizophrenia.Method:N400 detection was applied to 24 patients with bipolar depression(bipolar depression group),25 patients with unipolar depression(unipolar depression group),26 patients with schizophrenia(schizophrenia group)and 28 healthy adults(control group)with congruent ending words of Chinese idioms(matching)and incongruent ending words of Chinese idioms(mismatching)mode.Results:Compared with the control group,the N400 latency in the frontal area(Fz)was significantly delayed and the wave amplitude was decreased in the 3 patient groups(P<0.05 or P<0.01).In Fz brain area,N400 latency delay was positively correlated with general condition score(r=0.322,P=0.041).The decrease of N400 amplitude was negatively correlated with the positive symptom score(r=-0.398,P=0.007)and the total score of PANSS(r=-0.339,P=0.030).Conclusion:N400 can be used as one of the laboratories auxiliary references means to distinguish unipolar depression,bipolar depression and schizophrenia.This result needs to be further confirmed in the transformation.
