PTTG1、LncRNA H19的表达与早期声门型喉癌经口内镜CO_(2)激光切除术后远期疗效的相关性 被引量:2

Correlation between PTTG1,LncRNA H19 expression and long⁃term efficacy after transoral endoscopic CO_(2)laser resection for early glottic carcinoma

摘要 目的探讨垂体肿瘤转化基因(pituitary tumor transforming gene,PTTG1)、长链非编码RNA(long noncoding RNA,LncRNA)H19的表达与早期声门型喉癌行经口内镜CO_(2)激光切除术后远期疗效的相关性。方法选取2016年3月至2021年3月宜兴市人民医院收治的96例确诊为早期声门型喉癌并接受经口内镜CO_(2)激光切除术治疗的患者作为研究对象。检测入组患者的血清PTTG1及癌组织、癌旁组织中LncRNA H19表达水平,并收集患者的一般资料。对行经口内镜CO_(2)激光切除术的患者的远期疗效进行单因素分析后,应用Logistic回归分析分析风险因素与术后预期疗效的相关性。结果分析声门型喉癌患者血清PTTG1表达水平,根据淋巴结转移、肿瘤分期和分化程度对患者分组比较,差异有统计学意义(t=3.160,7.740,4.840;P<0.05)。分析组织中LncRNA H19表达,根据TNM(T是指原发肿瘤,N是区域淋巴结、M是指远处转移)、分化程度及淋巴结转移对患者分组比较,差异有统计学意义(χ^(2)=4.000,5.000,9.058;P<0.05)。喉癌显微激光手术疗效单因素分析,声门型喉癌浸润方向累及声门下区、分化程度及LncRNA H19和PTTGI表达水平比较,差异有统计学意义(χ^(2)=49.878,26.923,9.909,6.891,5.008,7.563,9.643,9.028;P<0.05)。Logistic回归分析结果显示,向下累及声门下区、高LncRNA H19、PTTG1表达水平均是影响早期声门型喉癌患者经口内镜CO_(2)激光切除术后远期疗效的独立预后因素(OR=13.649,95%CI:1.891~98.365;OR=12.466,95%CI:1.715~90.606;OR=11.045,95%CI:9.314~099;P<0.05)。结论声门型喉癌患者血清PTTG1、癌组织LncRNA H19表达水平均上调,这都与喉癌的发生发展密切相关,可作为早期声门型喉癌经口内镜CO_(2)激光切除术后疗效评估的参考指标。 Objective To investigate the relationship between the expression of pituitary tumor transforming gene(PTTG1)and long chain non‐coding RNA H19(LncRNA H19)and the long‐term curative effects of transoral endoscopic CO_(2)laser resection for early glottic carcinoma.Methods A total of 96 patients confirmed to have early glottic carcinoma treated with transoral endoscopic CO_(2)laser resection from March 2016 to March 2021 were enrolled as study subjects in Yixing People’s Hospital.The expression levels of serum PTTG1 and LncRNA H19 in carcinoma and paracancerous tissues were detected in the two groups,and general medical data of the patients were collected as well.After univariate analysis of the factors affecting long‐term efficacy after CO_(2)laser resection via oral endoscopy,Logistic regression was used to analyze the correlation between risk factors and expected efficacy after surgery.Results For early glottic carcinoma patients with lymph node metastasis,the higher the tumor stage and lower differentiation degree,the higher the expression of PTTG1(t=3.160,7.740,4.840;P<0.05).The higher the stage in the patients with high TNM in the LncRNA H19 high‐expression group(T refers to primary tumor,N refers to regional lymph node,and M refers to distant metastasis)and lower differentiation.The proportion of lymph node metastasis was all higher than that in the low‐expression group(χ^(2)=4.000,5.000,9.058;P<0.05).The infiltration direction of glottic laryngeal carcinoma involved subglottic region.For laryngeal carcinoma patients with lower differentiation and higher expression levels of LncRNA H19 and PTTG1,the lower the efficacy of laser microsurgery(χ^(2)=49.878,26.923,9.909,6.891,5.008,7.563,9.643,9.028;P<0.05).Logistic regression analysis showed that downward invasion of tumor affected the subglottic region.High LncRNA H19 and PTTG1 expression levels were all independent prognostic factors affecting long‐term efficacy of CO_(2)laser resection in the patients with early glottic carcinoma(OR=13.649,95%CI
