摘要
目的:研究血清基质金属蛋白酶-9(MMP-9)联合白蛋白、前白蛋白预测直肠癌根治术后早期吻合口漏的临床价值,为直肠癌根治术患者术后治疗提供临床参考依据。方法:选取2020年3月至2022年1月于本院确诊为直肠癌且采用直肠癌根治术的患者96例,根据其术后是否发生吻合口漏将其分为发生组20例和未发生组76例,比较2组患者血清MMP-9、白蛋白、前白蛋白水平,并对血清MMP-9、白蛋白、前白蛋白与直肠癌根治术后发生吻合口漏的关系进行分析。结果:吻合口漏发生组血清MMP-9在术后第3天、第5天、第7天均高于未发生吻合口漏组(P<0.05),血清白蛋白在术后第3天、第5天明显低于未发生吻合口漏组(P<0.05),血清前白蛋白在术后第1天、第3天、第5天、第7天均低于未发生吻合口漏组(P<0.05)。多因素Logistic回归分析结果显示血清MMP-9、白蛋白、前白蛋白均为直肠癌根治术后吻合口漏发生的独立危险因素。血清MMP-9联合白蛋白、前白蛋白预测直肠癌根治术后吻合口漏的曲线下面积(area under curve,AUC)高于血清MMP-9、白蛋白、前白蛋白单项检测。结论:直肠癌根治术后血清MMP-9联合白蛋白、前白蛋白预测早期吻合口漏具有一定的临床价值。
Objective To explore the clinical value of serum matrix metallo-proteinase-9(MMP-9)combined with albumin and pre-albumin in prognosticating early stoma leakage after rectal surgery for rectal cancer,so as to provide reference basis for managing stoma leakage after clinically radically treating rectal cancer.Methods Enrolled the 96 cases met above conditions treated in author’s hospital(2020-03-2022-01)into this study:according to with or without stoma leakage after surgery divided the patients into with one group(20 cases)and without one group(76 cases),compared both groups’levels of MMP-9,albumin and pre-albumin,further analysed the relation of the three factors to stoma leakage postoperatively happened.Results On the 3rd,5th and 7th day after surgery the MMP-9 levels of with one group was all higher than without one group(P<0.05);the levels of albumin on the 3rd,and 5th day after surgery in with one group were significantly higher than that in without one group(P<0.05),meanwhile after surgery on the 1st,3rd,5th and 7th day after surgery the levels of pre-albumin,lower(P<0.05).Multi-variate Logistic regression analysis showed above-mentioned three factors were all the independent risk ones;the combined detecting MMP-9,albumin and pre-albumin had higher AUC than single detecting MMP-9,or albumin,or pre-albumin.Conclusion After radical surgery for rectal cancer the combined detection of the three factors in prognosticating early stoma leakage has certain clinical value.
