摘要
目的探讨微创低温等离子甲状软骨板下切除术对累及前联合早期喉患者的影响。方法选取2016年2月至2021年2月江苏省宜兴市人民医院和江苏省镇江市第一人民医院72例累及前联合早期喉癌患者。按照随机数字表法分为观察组和常规组,每组36例。观察组采用微创低温等离子甲状软骨板下切除术治疗,常规组采用常规手术治疗。比较两组手术时间和术中出血量;比较两组发音功能;比较两组手术前后电视透视吞咽功能检查(VFSS)评分;观察两组并发症及复发情况。结果观察组手术时间短于常规组,术中出血量少于常规组(P<0.05)。观察组发音功能优于常规组(P<0.05)。术后3个月,两组VFSS评分高于术前,且研究组高于常规组(P<0.05)。观察组术后并发症发生率低于常规组(P<0.05)。观察组复发率低于常规组(P<0.05)。结论微创低温等离子甲状软骨板下切除术治疗累及前联合早期喉癌患者能有效改善发音功能及吞咽功能,安全可靠,可降低复发率。
Objective To investigate the effect of minimally invasive hypothermic plasma thyroidectomy on early laryngeal cancer involving anterior commissure.Methods A total of 72 patients with early laryngeal cancer involving anterior commissure admitted to Yixing People’s Hospital and Zhenjiang First People’s Hospital from February 2016 to February 2021 were selected,they were divided into observation group and conventional group,with 36 cases in each group.The observation group received minimally invasive hypothermic plasma thyroidectomy,and conventional group received conventional surgery.Operation time and intraoperative blood loss were compared between two groups;pronunciation function were compared between two groups;the scores of video fluoroscopic swallow study(VFSS)before and after surgery were compared between two groups;complications and recurrence were observed in two groups.Results The operation time of observation group was shorter than that of conventional group,and intraoperative blood loss was less than that of conventional group(P<0.05).The pronunciation function of observation group was better than that of conventional group(P<0.05).Three months after surgery,VFSS scores in both groups were higher than those before surgery,and observation group was higher than the control group(P<0.05).The incidence of postoperative complications in observation group was lower than that in conventional group(P<0.05).The recurrence rate of observation group was lower than that of conventional group(P<0.05).Conclusion Minimally invasive hypothermic plasma thyroidectomy can effectively improve the pronunciation function and swallowing function of patients with early laryngeal cancer involving anterior commissure,which is safe and reliable,and can reduce the recurrence rate.
