经皮穴位电刺激治疗中风后吞咽障碍疗效的Meta分析

摘要 目的:评价经皮穴位电刺激(Transcutaneous Electrical Acupoint Stimulation,TEAS)治疗中风后吞咽障碍的临床疗效。方法:计算机检索中国知网、万方、维普、中国生物医学文献数据库、The Cochrane Library、Pub Med等数据库,搜集自建库以来至2022年3月所有TEAS治疗中风后吞咽障碍相关临床随机对照试验研究。符合纳入标准的文献使用Rev Man 5.4软件进行Meta分析。结果:纳入13篇文献,共计896例研究对象。Meta分析结果显示,经皮穴位电刺激组(试验组)与对照组相比,总有效率[OR=3.18,95%CI(2.22,4.55),P<0.00001]、洼田饮水试验(Water Swallow Test,WST)[SMD=-0.89,95%CI(-1.10,-0.68),P<0.00001]评分、吞咽造影(Video Fluoroscopic Swallowing Study,VFSS)[SMD=1.17,95%CI(0.47,1.88),P<0.00001]评分、标准吞咽功能评定(Standardized Swallowing Assessment,SSA)[SMD=-5.11,95%CI(-8.81,-1.40),P<0.00001]评分的差异均有统计学意义。结论:TEAS治疗中风后吞咽障碍具有很好的临床疗效。由于纳入研究的数量及质量有限,该研究结论需要更多高质量的研究的支撑。 Objective:To evaluate the clinical efficacy of transcutaneous electrical acupoint stimulation on dysphagia after stroke.Methods:In CNKI,Wanfang,VIP,Sinomed,The Cochrane Library,PubMed,databases,all clinical randomized controlled studies related to the treatment of dysphagia after stroke by transcutaneous electrical acupoint stimulation from the establishment of each database to March 2022 were collected to perform a meta-analysis of the literature that met the inclusion criteria by RevMan 5.4 software.Results:A total of 13 literatures were included,with 896 subjects.The results of the meta-analysis showed that there were statistically significant differences in the total efficiency,water swallow test(WST)score,video fluoroscopic swallowing study(VFSS)score,and standardized swallowing assessment(SSA)score between the control group and the percutaneous acupoint stimulation group.Conclusion:Percutaneous acupoint electrical stimulation shows good clinical efficacy in the treatment of dysphagia after stroke.However,due to the limited number and quality of included studies,more high-quality research is needed in the future to support this conclusion.
