超声电磁导航定位穿刺引导设备在穿刺活检中的可行性及准确性研究 被引量:1

摘要 目的:探讨超声电磁导航定位穿刺引导设备引导穿刺活检的可行性及准确性。方法:利用生物胶体及仿生硅胶皮制作仿生组织透明体模,内含30个球形靶点(直径10 mm,深度60~80 mm),由1名有介入超声经验的医师(医师A)和1名无介入超声经验的医师(医师B)进行操作,两名医师对30个球形靶点分别采用超声电磁导航定位技术、超声徒手穿刺及超声引导架3种方法各活检一次,对比3种引导方法的穿刺时间、活检成功率及获取目标的有效长度。结果:医师A使用超声电磁导航定位技术获取目标的有效长度和操作时间均优于超声引导架及超声徒手穿刺,其差异均有统计学意义(Z_(有效长度)=-4.20,Z=-4.40;Z_(操作时间)=-6.61,Z=-4.90;P<0.05)。医师B使用超声电磁导航定位技术的活检成功率高于超声徒手穿刺,其差异有统计学意义(χ^(2)=12.0,P<0.05);使用超声电磁导航定位技术获取目标的有效长度和操作时间均优于超声引导架及超声徒手穿刺,其差异有统计学意义(Z_(有效长度)=-6.19,Z=-4.20;Z_(操作时间)=-4.40、Z=-6.12;P<0.05)。两名医师使用超声徒手穿刺活检的成功率差异有统计学意义(χ^(2)=3.57,P<0.05);使用超声徒手穿刺活检获取目标的有效长度差异有统计学意义(Z=-3.13;P<0.05);在使用超声引导架穿刺活检的操作时间、获取目标的有效长度差异均有统计学意义(Z=-3.41,Z=-2.36;P<0.05)。医师A与医师B在使用超声电磁导航定位技术的活检操作时间、成功率及获取目标的有效长度差异均无统计学意义。结论:超声电磁导航定位技术具有较高的准确性,尤其是对于无介入操作经验的医生也具有很好的可重复性和准确性,有利于提高操作者的信心及操作安全性。 Objective:To investigate the feasibility and accuracy of guided equipment of puncture with location of ultrasound electromagnetic navigation in guiding puncture biopsy.Methods:The transparent phantom of biomimetic tissue was made by using biological colloid and biomimetic skin of silicone rubber,containing 30 spherical target spots(diameter 10 mm,depth 60-80 mm).The operation was respectively conducted by physician A with interventional ultrasound experience and physician B without interventional ultrasound experience.The two physicians respectively used the location technique of ultrasonic electromagnetic navigation,puncture with hand under ultrasound and ultrasonic guide frame to conduct biopsy for 30 spherical target spots.The time of puncture,the success rate of biopsy and the effective length of obtaining target among three guidance methods were compared.Results:The effective length and operation time of physician A using the location technique with ultrasonic electromagnetic navigation in obtaining target were significantly better than those of ultrasonic guide frame and puncture with hand under ultrasound,and the differences between them were statistically significant(Z_(effective length)=-4.20,Z=-4.40,Z_(operation time)=-6.61,Z=-4.90,P<0.05),respectively.The success rate of physician B using the location technique with ultrasonic electromagnetic navigation was significantly higher than that of puncture with hand under ultrasound,and the difference between them was statistically significant(x2=12.0,P<0.05),and the effective length and operation time of physician B using the location technique with ultrasonic electromagnetic navigation in obtaining target were significantly better than those of ultrasonic guide frame and puncture with hand under ultrasound,and the differences between them were statistically significant(Z_(effective length)=-6.19,Z=-4.20,Z_(operation time)=-4.40,Z=-6.12,P<0.05),respectively.There was statistically significant difference in the success rate of the biopsy of using puncture wit
