单细胞计数法用于革兰阴性菌药敏检测的性能比对研究

摘要 目的:评估单细胞计数法革兰阴性杆菌药敏板用于临床常见革兰阴性菌药敏检测的性能。方法:选取北京和上海地区两家三甲医院分离出的364株革兰阴性菌,其中肠杆菌目菌284株,非发酵菌80株,使用单细胞计数法革兰阴性杆菌药敏板(考核试剂)进行药敏试验,采用革兰阴性菌GN4F药敏板、肉汤微量稀释法药敏试剂和左氧氟沙星E-test药敏条3种药敏检测试剂作为对照试剂进行药敏试验,与考核试剂的检测结果与对照试剂的结果进行比较,评估考核试剂中21种药物的药敏检测结果分类一致率(CA)、基本一致率(EA)、重大偏差(MD)和极重大偏差(VMD)。结果:单细胞计数法革兰阴性杆菌药敏板2~4 h即可判读结果,所评价的21种药物的整体CA和EA均≥90%,VMD和MD均<3%,在黏质沙雷菌和枸橼酸杆菌的药敏检测中表现最好。在肺炎克雷伯菌、奇异变形杆菌、阴沟肠杆菌、产气克雷伯菌、沙门菌属菌、大肠埃希菌、普通变形杆菌和产酸克雷伯菌中的药敏结果表现良好,存在不可接受情况的药物均控制在两种以内。而摩氏摩根菌和普罗威登斯菌对头孢他啶等多种药物、以及部分非发酵菌对于美罗培南或左氧氟沙星的CA和EA均<90%。结论:单细胞计数法革兰阴性杆菌药敏板以微量肉汤稀释法为基础,用于革兰阴性菌药物敏感性检测具有临床使用兼容性高、速度快和灵敏度高的优势。 Objective:To assess the performance of Gram-negative bacteria antimicrobial susceptibility testing plates based on single cell counting method in the test of antimicrobial susceptibility of clincallly common Gram-negative bacteria.Methods:A total of 364 gram-negative strains,including 284 strains of Enterobacteriaceae and 80 strains of non-fermentive bacteria,were isolated from two class A tertiary hospitals in Beijing region and Shanghai region.Antimicrobial susceptibility tests were performed using Gram-negative bacteria antimicrobial susceptibility testing plates based on single cell counting method as test method,and commercial antimicrobial susceptibility reagents as control reagents,including GN4F antimicrobial susceptibility plate of Gram-negative bacteria,antimicrobial susceptibility reagent of microbroth dilution method and levofloxacin E-test antimicrobial susceptibility strip.The tested results of checked reagent were compared with those of control methods so as to assess the category agreement(CA),essential agreement(EA),major discrepancy(MD)and very major discrepancy(VMD)of the tested rates of antimicrobial susceptibility of 21 kinds of medices of the checked reagents.Results:The results could be interpreted by Gram-negative bacteria antimicrobial susceptibility testing plates based on single cell counting method after 2-4 hours,and the whole CA and EA of the evaluated 21 kinds of medicins were larger than or equal to 90%,and the VMD and MD were less than 3%,the performances of them were favorable in the test of antimicrobial suspceptibitlies for Seriferia marcis and Acidobacter citrate.The performances of the results of antimicrobial suspceptibilies also were favorable for Klebsiella pneumoniae,Proteus mirabilis,Enterobacter cloacae,Klebsiella aeromogens,Salmonella,Escherichia coli,Bacillus proteus vulgaris and Klebsiella oxytoca.The medicines that excisted unacceptability were controlled within two kinds.However,the CA and EA of Morgan Morganella and Provencia sp were less than 90%for many kinds of
