摘要
目的探讨声触诊弹性成像(STE)在评估巴德-吉亚利综合征(BCS)短期疗效中的应用价值。资料与方法回顾性收集2020年7-12月在徐州医科大学附属医院诊断为BCS的患者30例,根据肝功能Child-Pugh分级分为A级19例、B级8例、C级3例,患者术及前术后均接受STE检查,评价STE检测可重复性,对比肝、脾硬度值在不同肝功能组及患者治疗前后的变化,并分析其与肝功能Child-Pugh分级、肝静脉压力梯度(HVPG)的相关性。结果所有患者均成功接受治疗,术前不同肝功能分组(A~C级)的肝硬度值[(11.53±2.18)kPa比(22.16±6.23)kPa比(36.16±4.67)kPa]、脾硬度值[(19.01±4.81)kPa比(27.76±9.84)kPa比(28.46±13.51)kPa]差异有统计学意义(F=62.53、4.84,P均<0.05)。肝、脾硬度平均值与肝功能Child-Pugh分级呈正相关(r=0.904、0.466,P均<0.01)。术前及术后肝硬度值[(16.83±8.89)kPa比(12.69±6.09)kPa]、脾硬度值[(22.19±8.27)kPa比(15.97±5.41)kPa]、HVPG[(13.03±4.54)mmHg比(6.23±3.10)mmHg]均降低,差异有统计学意义(t=5.105、7.833、16.445,P均<0.001)。术前术后肝、脾硬度差值与HVPG差值呈正相关(r=0.561、0.578,P均<0.001)。结论使用STE进行肝、脾硬度检测与肝功能Child-Pugh分级具有较好相关性,且对门静脉压力变化较敏感,操作可重复性较好,在评估BCS患者短期疗效中有一定应用价值。
Purpose To investigate the value of sound touch elastography(STE)in the evaluation of short-term curative efficacy of Budd-Chiari syndrome(BCS).Materials and Methods A total of 30 patients diagnosed with BCS from July to December 2020 in the Affiliated Hospital of Xuzhou Medical University were retrospectively recruited and divided into three groups according to Child-Pugh grading of liver function(including grade A=19 cases,grade B=8 cases,grade C=3 cases).Patients underwent STE examination before and after treatment to evaluate the repeatability of STE detection.The differences in liver and spleen stiffness values before and after treatment and in different Child-Pugh grading of liver function groups were compared and the correlation with Child-Pugh grading of liver function and hepatic venous pressure gradient(HVPG)were further analyzed.Results All patients were successfully treated.There were statistically significant differences in liver stiffness values[(11.53±2.18)kPa vs.(22.16±6.23)kPa vs.(36.16±4.67)kPa;F=62.53,P<0.001],spleen stiffness values[(19.01±4.81)kPa vs.(27.76±9.84)kPa vs.(28.46±13.51)kPa;F=4.84,P=0.016]among different liver function groups,respectively.The stiffness of liver and spleen was positively correlated with Child-Pugh grading of liver function(r=0.904,0.466,both P<0.01),respectively.Compared to the before treatment,the liver stiffness values[(16.83±8.89)kPa vs.(12.69±6.09)kPa],spleen stiffness values[(22.19±8.27)kPa vs.(15.97±5.41)kPa]and HVPG[(13.03±4.54)mmHg vs.(6.23±3.10)mmHg]were significantly lower than those in the after treatment with statistically significant differences(t=5.105,7.833,16.445,all P<0.001),respectively.The difference of liver and spleen stiffness before and after treatment was positively correlated with HVPG difference(r=0.561,0.578,both P<0.001).Conclusion Using STE to detect liver and spleen stiffness has a good correlation with Child-Pugh grading of liver function.STE is sensitive to changes in portal vein pressure,and has good repeatability,which
